3.0 MACHINERY & EQUIPMENT
4.0 RESPONSIBILITIES
• It is overall responsibility of Project Manager to organize resources prior to perform construction activities as per project specification in compliance with the quality, schedule & safety requirements
• EHS Manager will ensure in coordination with Site Engineer that all measure/construction taken shall
be maintained till completion of job
• It is the responsibility of Construction Manager/Site Engineer that construction activities are executed

• DMPP will ensure that all works are performed safely according to the attachment
• The relevant DMPP Supervisor will ensure that the work is carried out in accordance with this method statement and project specification
• The QC Inspector will ensure that the work is executed according to the requirements specified in the design drawings and MS and ITP
• The QC Inspector will ensure that material before installation, certificate of material shall be checked prior to installation
5.0 PROCEDURE AND SEQUENCE
5.1.1 ANCHOR BOLT SETTING
Step 1: Marking for boundary line of starting column on footing
Step 2: Installation and fixing for Anchor bolt Frame shall be followed design drawing as such center line& elevation…
Step 3: Marking for bolt-group-center line on side of form
Step 4: Set template with anchor bolts The template position is higher than concrete surface level Making line of bolt group center and each bolt must be matched on the template Adjust position of the template according to making line on footing by plum bob
Step 5: Welding diagonal supporting bar to method steel bar and anchor bolt
Step 6: Set form for starting column and support them carefully

The Site Inspection & Test Plan (ITP) for this work summarizes various characteristics to be checked The concerned Site Engineer/Foreman will be responsible to ensure stage wise compliance for this operation and the Site QA/QC Engineer/Inspector will carry-out quality control checks and report the inspection results All workers are trained to perform, certified skilled for this work and the QC Engineer will tightly control on the site
7.0 ENVIRONMENT, HEALTH AND SAFETY (EHS)
7.1 Responsibilities and Instructions
• PM/EHSS Officer shall ensure that all work is undertaken in accordance with the specified work scope
• Safety training should be conducted to all personnel assigned at site prior to commence any work
• All personnel should know the locations of fire extinguishers, communications
• All personnel should be aware of assembly points, escape routes, emergency contact numbers
• Carry-out daily tool box and safety talks
• Personal Protective Equipment (PPE) shall be worn at all times
• Good housekeeping shall be observed in work areas
• Construction wastes shall be segregated and disposed to an approved dump yard area
• Permit to work shall be obtained appropriately for the type of work and location
• Adequate lighting equipment when working night time
• Ensure all workers and equipment working overtime are properly accounted and obtained permit/approval from DMPP EHS Manager prior to carry-on works
• Periodic refresher trainings, toll box talk and audit shall be conducted
7.2 Safety( PPE )
Trang 7• Personal Protective Equipment (PPE) will be issued to all workers such as safety shoes, hard hat, working gloves, earplug, safety goggles, reflective vest and safety harness for workers who will be assigned in elevated places 1.8m and above
• Wearing of the basic PPE will be enforced fully
• All new recruits will undergo proper orientation and safety induction before they could deployed at site for work
