I) Aims :
- Help students to improve their listening, reading and speaking skills.
- Have a further practice in present /past simple tense and the structure “Would you + V…?”.

II) Objectives:
By the end of the lesson, students will :
have learnt how to read and fill out a medical form
have been able to follow / respond to doctor’s instruction during a check-up
have practiced the structure “Would you + Verb” in correct way
have talked about one’s height, weight
