Chọn BN :

• Đau thắt lưng – chậu : 1, 2 bên

• Lan ra vùng mông(94%) thắt lưng(50%),

đùi(50%),bẹn(14%)

• Đau khi ngồi lâu ( có thể ngồi 1 bên)

• Tê , ê ẩm , châm chích vùng mông và đùi

• Không triệu chứng rễ

• Không có bệnh lý khớp

• Patrick’s test , Flamingo test, Gaenslen’s test…

• Chẩn đoán hình ảnh : ít giá trị

Đau lưng 1 , 2 bên lúc nghỉ, mọi tư thế

Sâu , âm ỉ vùng lưng , lan ra mông, hông , đùi ( không theo rễ) , không đau gối

Đau khi xoay người, ưỡn người> cúi người Đau có thể giảm bớt khi gập người lại

Đau tăng lên khi đi ,đứng ,ngồi lâu

Morning stiffness

Không dấu hiệu TKNB

Sờ vùng lưng đau ( tenderness) tương ứng vùng facet Không đau rễ

• Bó mạch thần kinh nằm trong khe ngay dưới

bờ dưới xương sườn , nằm ở giữa lớp cơ liên sườn trong và lớp cơ liên sườn trong cùng

• TKLS có 4 nhánh : lưng , tạng ,bì bên ,bì trước

• T12 : không chạy theo bờ sườn > tê TAP , Tê

Chỉ định

• Gãy xương sườn

• Đau TK LS do Herpes Zoster ( cấp ,mạn)

• Giảm đau hậu phẩu

• Đau TKLS vô căn

• Chẩn đoán

• Nhiễm trùng toàn thân ,tại chổ

• Thay đổi cấu trúc giải phẩu ( cắt xương sườn)
