1.0 General Description of Works / Dates of Operation Date : 19-06-2015

Tower Crane TC: LTP200/10-5032

TOWER CRANE WILL BE USED TO LOAD /OFFLOAD, RE-LOCATE [WITHIN THE SITE

BOUNDARY] VARIOUS PIECES OF EQUIPMENT WITHIN THE CRANES RADIUS

 Offloading/loading construction materials to/from delivery vehicles

2.0 Tower Crane details

Monthly inspections will be submitted prior to usage and kept on file in Deutsches Haus site office, monthly lifting accessories inspections will also be kept in Deutsches Haus site office

3.0 Description of Loads (see section 6 for slinging details)

Description of loads (Weight, characteristics, method of lifting, centre of gravity and lifting points A LIFT ASSESSMENT MUST BE DONE FOR ALL COMMON LIFTS COMPLEX LIFTS MUST HAVE SEPARATE LIFT PLAN APPROVED LIFT ASSESSMENTS LISTED BELOW Copies attached

5.0 Lifting Accessories [TC]

10.0 Details of lifting area demarcation to be used & how will access be

controlled

To be determined at point of lift by Crane Co-ordinator, no un-authorised personal will be allowed within the Lifting Zone whilst works are in progress

Banksman will ensure the safe passage of movement at all times by staying in direct line of sight of the crane operator were possible and maintain radio contact at all times

The area for loading and unloading will be agreed prior to lifting operation commencing The load will arrive at an agreed time nd will be positioned by the Banksman

Prior to lifts the Lift Supervisor will ensure that the crane accessories are fit for purpose with a visible inspection

11.0 How will the suspended load be controlled? (e.g tag line etc)

Tag-lines will be used to control the lift and will be fitted prior to lifting

Maximum safe wind speed for Lift (taking into account load characteristics):13m/s

These are guide lines only and the crane driver will make final decision depending on daily characteristics

17.1 Tower Crane Rescue

HBC are in control of all Tower Crane Operations, therefore all Rescue Arrangements are to

be implemented by HBC

Fully Trained Operatives in Tower Crane Rescue must be present on site at all times whilst Tower Cranes Operations are taking place

17.2 PPE

The following PPE is to be worn to comply with HBC/Mace requirements

17.3 Tower Crane Rescue/Security

Lifting supervisor/ Safety manager keeps the main gate keys to prevent any unauthorized person access to TC footing area

PLEASE NOTE-KEYS MUST BE RETURNED AFTER USE AND REGISTER UP-DATED, REGISTER TO BE UP-DATED WHEN TAKING KEYS BEFORE SHIFT STARTS
