1 Safety Policy

1.1 Safety Organization and Allocation of Responsibility

Hoa Binh safety organization comprises the Safety Section, of which the primaryfunction is to provide direction and leadership to the Project Safety Committee toimplement the Safety Management System for this project

Functions of Safety Section

 Formulation and implementation of the Project Safety Management System(SMS).Promote enthusiasm & guidance to keep the organization's safety andhealth program active and productive

 Approving body for the use of all safety equipment & Conduct accident

investigation and safety inspections

 Recommend safety measures for implementation & Responsible for first aid facilities

 Administer the penalty system for recalcitrant contractors I workers

Constant review and updating or the Project SMS and other relevant safety standards' guidelines

Organization Structure

Members of the Safety Section comprise the Project Manager, ConstructionManager, Safety Officer , Safety Supervisor & Sub-Contractors

Chairman, Safety Committee

 The Project Manager shall resume this position

 Overall Management of Safety Section & Act as Head of Safety Section

Duties and Responsibilities are:

 Too familiar with the company's safety policy and program and ensure its effective application and implementation at his worksite

 To give leadership and direction in the daily administration of site safety

activities including fair response to safety recommendations and act on it promptly

 To take prompt corrective actions to remove any unsafe or unhealthy workconditions and be responsible to maintain effectively such correction made

 To ensure that good housekeeping are practiced on site and in any cases ofaccidents and dangerous occurrence, to help and co-operate in theinvestigation with the assistance of safety officer I safety supervisor for thepreparation of investigation report

 To provide safety Officer’s and safety Supervisor’s with facilities, equipmentand information necessary to enable him to promote the safe conduct ofwork within the company’s sites

Duties and Responsibilities are:

 To review all meetings of the safety committee and give full support to allcommittee activities including reviewing all work methods and process, bothnew and existing to see that all safety precautions are met.

 To provide all safety measures and equipment necessary for the prevention ofaccident

 Responsible for the provision of a safe and healthy working environment for allpersons working at the worksite

Deputy Chairman, Safety Committee

 The Construction Manager shall assume this position

 Assist the Chairman of Safety Committee on all matters relating to safety andhealth

 In the absence of the Chairman, he shall lead the Safety Section and chair theMain Safety Committee

Work Place Safety & Health Officer

 Report to the Chairman I Deputy Chairman, Safety Committee & Conductdaily safety inspection and compile daily and monthly safety reports

 Implement safety and health standards I regulations formulated for theworksite & Investigate all accidents I dangerous occurrences and correctcauses to prevent future occurrence

 Safety enforcement on all site personnel & maintain the site safety records &Coordinate with the various sub-contractors pertaining to safety and health ofthe workplaces

 Promote safety awareness among sub-contractors and workers

 Recommend safety measure to all site personnel including engineer’ssupervisors I foremen, subcontractors and nominated subcontractors forimplementation

 Attend Safety Committee meetings & Implement safety training and campaignprograms

 Initiate investigation into all accidents/dangerous occurrences andrecommend corrective or preventive measures to prevent recurrence.Evaluate accident statistics and review overall safety performances

 To conduct safety audit and inspection of site with Project Management Team

to ensure compliance to safe work practices and work activities are carriedout in a safe manner

 To advise relevant sections/trades to maintain relevant reports, records andcertificates required under the Workplace Safety & Health Act andRegulations

 Supervise the work of safety supervisors, and advises scaffoldsupervisors and lifting supervisors as required on the project

 To suspend any operation that constitutes a breach of any HSE legislation, or

 Promote safety awareness among sub-contractors and workers.

 Recommend safety measure to all site personnel including engineer'ssupervisors I foremen, subcontractors and nominated subcontractors forimplementation

 Attend Safety Committee meetings & Implement safety training and campaignprograms

 General safety and health correspondences & supervise the erection of safetyfeatures

 Any other safety and health related duties that are assigned by themanagement

1.2 Project Safety Committee

The main role of the Project Safety Committee is to implement the SafetyManagement System for the worksite The Committee shall compriserepresentatives from all subcontractors, nominated subcontractors, and foremen,including key representatives from the management, i.e engineers, supervisorsand safety supervisors The meeting shall be chaired by the Project Manager or inhis absence, the Deputy Project Manager, and shall convene at least once amonth The duties of the Project Safety Committee members are:

The Safety Committee Membershall:

 Inspect work areas at least once a week to see that if there is anythingprejudicial to the safety or health of the persons employed therein Anypotential safety, fire or health risks shall be referred to the Safety personnel,Supervisors I Managers immediately

 Attend monthly safety committee meeting to discuss the unsafe acts andconditions observed by the members during the course, of work and torecommend necessary safety measures

 To assist in the investigation into any accident or dangerous occurrence anddiscuss the causes and measures to be taken to prevent the recurrence ofsuch accident or dangerous occurrence

Project Engineers and Site SupervisorsI Foreman:

The Project engineers and their site supervisors I foremen shall assist the Project Manager to achieve the objectives of the Safety Policy

Their duties and responsibilities are:

Duties of Site Engineers:

All site engineers shall assist the Project Manager in achieving theobjectives of the Safety Management System

 Enforce on compliance of the TCVN, HSE, OSHA, MOM Act & Regulations onwhichever is reasonably practicable and its subsidiary legislation and theSafety Management System by subordinates, subcontractors and contractors

or their representatives under their control;

 Ensure that all subordinates and sub-contractors or their representatives undertheir control comply with the safety policy;

 Ensure that sub-contractors or their representatives investigate into causes ofevery accident and dangerous occurrence & attend meetings convened for thepurpose of accident prevention

Duties of Site Engineering and Technical SupervisorsI Foreman:

Site supervisors and foremen shall be responsible for assisting the Site Engineer

in implementing and enforcing the safety policy in accident prevention

They shall:

 Enforce on compliance of the TCVN, HSE, OSHA, MOM Act & Regulations

on whichever is reasonably practicable and its subsidiary legislation and theSafety Management System by subordinates, subcontractors and contractors

or their representatives under their control;

 Check and ensure that all necessary tools and equipment are safe to use;

 Ensure that all sub-contractors provide and maintain safe access in all places

of work

 Ensure that all sub-contractors maintain the highest standards of housekeeping

in their work areas;

 Assist in investigating and reporting on accidents and dangerous occurrencesand take prompt remedial action as recommended

 Ensure that all sub-contractors do not deploy new or transferred worker toundertake any tasks without appropriate instruction;

 Ensure that the necessary protective clothing and safety equipment is readilyavailable and that contractors provide workers with the necessary personalprotective equipment;

 Ensure that all sub-contractors strictly enforce on the use of all necessaryprotective clothing and safety equipment at all time by workers;

 Attend all safety committee meetings and any other meetings required for thepurpose of accident prevention;

 Set the example for all workers to follow at all times; and observe strictly to thesafety advice of the safety supervisor

1.3 Subcontractor’s

All sub-contractor and nominated sub-contractor who employs more than 20workers shall either himself be trained and qualified in .construction safety

Duties of Subcontractor’s & Nominated Subcontractor’s:

 Responsible for the safety of their workers

 Ensure that all their workers have undergone Safety Orientation Courseconducted by MOLISA (Ministry of Labor, Invalids and Social Affairs) andClient prior to deploying them for work

 Responsible for obtaining prompt first aid or medical attention to injured

 Conduct daily safety I housekeeping inspection of work area and rectify allunsafe acts I conditions

 Accompany the safety supervisor I engineer in the safety inspection of anywork area

 Carry out all safety measures recommended by the Safety Section promptly

 Report and investigate all accidents I dangerous occurrence and correctcauses A report of such shall be submitted to Safety Section for information

 Hold regular safety discussion with his worker

 Carry out all safety recommendations or measures as suggested by theSafety Section and their safety supervisors

 Ensure themselves and all their workers adhere strictly to the site's safetyregulations and the Factories (Building Operations and Works of EngineeringConstruction) Regulations

 Shall engage qualified Full time safety supervisor to supervise their workerworking at site

1.4 Supporting Stafff or Safety Section:

Project DirectorI General Manager

Project Director shall give leadership and direction and total commitment of safety

at the worksite and his duties and responsibilities are

 To ensure the total commitment of the entire organization to achieve safe design, safe operation and maintenance; back up adequate safety training, discipline and motivation

 To give leadership and direction in the administration of safety activities at thetop management level

 To provide safety personal with facilities, equipment and information

necessary to enable him to promote the safe conduct of work within the

company's site

 To afford the essential facilities for the proper implementation of safety

program and ensure that safety is planned into all operation

First-Aid / Industrial Nurse Personal

 Report to the Site Safety Officer

 Maintain the first aid facilities and attend to all injured personnel in the worksite

 Provide basic first aid knowledge and training to site personnel

Clerical Assistant (Safety Administration)

 Assist the Safety Officer I Safety Supervisor in all typing and correspondence works

Safety Erectorsor Promoters (Safety Workers)

 Report to the site safety supervisor & Assist in the safety enforcement andinspection.

 Erect physical safety features e.g barricading, overhead shelter etc

 Recommended that 2 safety promoters at the initial stage of work When theconstruction progress, then more promoters may be required

Lifting Supervisor

 Co-ordinate all lifting activities;

 Check the load and riggings to ensure that the load can be lifted and removedsafely by any crane, mobile crane or tower crane;

 Ensure that the ground conditions are safe for any lifting operation to beperformed by any crane, mobile crane or tower crane;

 Ensure that any lifting activities are carried out in accordance with safe andsound practice;

 Take all necessary measures to rectify any unsatisfactory or unsafeconditions reported to him by the crane operator;

 To monitor daily checking of crane and passenger hoist by operators &ensure the maintenance and inspection program of crane and passengerhoist;

 To ensure rigger/slinger, signalman, crane operator carry out work safely andpromptly & check the load and riggings to ensure that the load can be liftedand removed safely by any crane

Rigger & Signalman

 Ensure that the loads are secured and it is safe to be lifted

 Must have qualified training certificate & Wear all proper PPE and useapproved attire at all times

 Ensure the lifting gears, web slings, wire ropes, shackles and hooks are

as manufacturer recommended and in good working conditions

 Ensure the lifting operation shall be carried out safely and the cranemovement free from obstruction

 Ensure that the crane operator shall only take the signal from signal man only

 Ensure the safety of the other personnel at site & Report anyunsafe conditions to the lifting supervisor

 Stop all lifting activity if there is any change in the weather conditions/ raining

Forklift OperatorI Scissor Lift & Boom Lift

 Must have a valid qualification certificate I approved training to operate theforklift

 Ensure that the operation does not expose any danger to site personnel andthe environment

 Check all the safety devices of machinery before use and ensure it is safe touse

 Hanger the safety harness to the anchoring point.

 To erect stairs or ladders for the person to gain access from one level toanother level

 Ensure the scaffold does not pose any danger to all site personnel duringerection modifying & dismantling

 To ensure no scaffold shall be erected or modified or dismantledexcept under immediate supervision by the scaffold supervisor

 Brief the safety aspect before erection, modification or dismantling of scaffold

 Inspect all the scaffolding material before use, if there are any damage partsdon't use

 To erect stairs or ladders for the person to gain access from one level toanother level

 Ensure the scaffold safe to use & conduct the inspection every 7 days

 Inspect the scaffold immediately after heavy rain or any change in the weatherconditions is likely to affect the strength of the scaffold components

Explosive Power Tool Operator

 Must have a valid license to operate the power tools I ramset gun etc

 Ensure that the operation does not expose any danger to user and all sitepersonnel

 Check the condition of the power tools before use & to safeguard the ramsetgun

 Do not modify any safety devices

All Staffs, Workers, Suppliers & Visitors

 Wear all basic Personal Protective Equipment (PPE) before enter to the site

 Use appropriate PPE as required by the nature of work & observe all Safety

requirements and report any un- safe condition to safety personnel.

 Attend as safety training & toolbox meeting as required & Display the securityaccess pass at all times when working at site

 Follow the site rules and regulation displayed in the safety notice board

1.5 Project Safety Team Organization Chart

The safe work procedure applied at all workplace to eliminate or minimize the risk

of injury to people, damage to property and harm to the environment

All works shall be carried out by appropriate trained, competent, approved,qualified or licensed persons in accordance with the applicable Code of Practiceand Regulations as laid down by MOC, MOLISA, HSE, OSHA, MOM and otherrelevant Authority These procedure should be documented as work procedure orinstruction in case where is statutory requirement to do so or in other cases where

it is deemed necessary

2.2 Scope

The following are the Safe work procedures are required in compliance with thestatutory requirement; MOLISA, HSE, OSHA, MOM and the best Code ofPractices and Standards for the scope of work shall apply However the followingsafe work procedure serves only as guide

 Installation of pipe supports, pipes & Structural Steel Erection

 Working at heights

 Scissor Lift & Fork Lift

 Electric welding & Laying of cables

 Hot works

 Electrical works

 Excavation

 Erection and Dismantling of Scaffold etc

2.3 Procedure

All contractors must set a safe work procedure for their work on site and the

contractors are to follow all work instruction required by and the highest and inclusive of Where Reasonably Practicable shall be in conformance to the below Safety ACT, Regulations, Approved Code Of Practice and guidelines and shall be accompanied with the proper documentations and relevant drawings with

endorsements from a Certified and licensed Professional Engineer;

Trade supervisor or foreman shall instruct any worker who performs unsafe act orcarries out work not in accordance with the appropriate code of practice to correctthe unsafe act or improper practice

Safety officer or site safety supervisor who observes the performance of unsafeact or work practice by any person at the worksite shall stop such performanceimmediately by either giving direct instruction to the offender or through thesuperior of the offender

2.5 Permit – To – Work System

Permit-to-work is required to perform the works as stated below:

4.0 Group Meetings

4.1 Objective

The objective of the group meetings are held for the purpose of effective safetycommunication, coordination and control of the Project Safety ManagementSystem It is to be held regularly and to involve personnel of all levels Attendance

is to be recorded to reflect individual I group commitment, and minutes of meetingsproperly documented and circulated for the necessary follow-up action Groupmeetings will help to promote safety issues, integrate participation at all levels, andpromote group cohesiveness, cooperation and commitment Leaders must set theexample, and ensure that the level of interest and commitment is sustained

throughout the project duration.

4.2 Purpose

To communicate, review, discuss, and establish safety Policies and proceduresapplicable to the works by conducting the following

4.3 Monthly Safety Committee Meeting

The purpose is to implement, control and coordinate safety management system.The Project Manager will chair the meeting with the safety supervisor I consultant

as secretary Committee members include company staff and sub-contractorsrepresentative

4.4 Monthly Safety Talk at Assembly Area

The safety talk is to promote safety policy, rules and regulations and safetycampaign The Project Manager I Safety Officer I Contractor Safety Personnel willspeak to all workers based on relevant safety topics

4.5 DailyI WeeklyTool-Box Meeting

Tool-box meetings are conducted Daily/weekly to facilitate safety communication atworkers' level The respective trade supervisor/foreman will conduct the meetings.The tool-box meeting records will be submitted to the Voltas Safety Personnel byweekly

4.6 Site coordination Meeting

The purpose is to identify daily safety discrepancies on site e.g housekeeping,safety status and implement corrective actions Safety coordination for followingday's work will also be discussed in the meeting The project team andsubcontractors will attend the meeting

4.7 Pre-construction meeting with subcontractors before commencement

of work

The purpose of the meeting is to brief the safety requirement of the owner Iconsultant and main contractor of the worksite and to ensure the contractor has theproper track record on their scope of work

5.0 IncidentI Accident Investigation Analysis

Incident or Near Miss

 Specific event or condition that has and impact on safety or health of people,

on property, or on legal and regulatory compliance

 All incident and near miss are reportable and require notification to linemanagement

 A near miss is an unintentional or unwanted event or circumstances whichunder slightly different conditions would result in an accident

An Incident is an unplanned event or occurrence, which results in:

 Failure or collapse of Cranes, Hoist, Winch or other appliances rising or

lowering goods or persons

 Collapse of Support & Structure

 Electrical Short Circuit or failure of plant machinery or apparatus & Explosion

or Fire

5.3 Reporting of Accident / Incident

 All Incident I Accident I Near misses must be reported immediately to theimmediate supervisor who will inform the safety officer, project manager and tothe Client safety personal on duty

 All employees shall report immediately to their immediate supervisor if anyincident occurs at work site

 First Aider I Nurse shall give first aid treatment to the injured person, if theinjuries are minor the Safety personal shall arrange for the injured to see thecompany doctor or to the closest Hospital

 Incident Report: A preliminary report must submit for Incident I Accident I Nearmiss to the Client safety manager within 6 hours

 A detailed report of Accident I Incident shall be submitted to the safetymanager within 3 days The report shall enclose the location, date & time,nature of accident, details of injured person, copy of investigation report andrecommendation and action plan for preventing a recurrence

 For serious injuries the project manager I safety personal call the ambulanceand the immediate supervisor shall accompany the injured person to hospital

If necessary inform the relevant authorities

5.4 Scope and Level of IncidentI Near miss Investigation:

Based on the severity or potential consequences and legal consideration ofincident or significant near miss, the project manager /safety officer will decide onone of the following levels

Level 1: Initial investigation by the line supervisor and appropriate immediate

corrective action taken No further action is required

Level 2: May required an internal investigation.

Level 3: Required investigation by external party

5.5 Levels of Investigation

An Incident investigation committee will be appointed base on the seriousness ofthe incident/near miss within 4 hours of the incident or immediate follow up to beimplement The different levels of investigation are to ensure that;

 Root causes and contributing factors are identified

 Action needed to reduce the risk of this related incidents are determined

 Appropriate action is taken and documented and Investigation is timely

5.6 Incident Investigation Team

An incident investigation team shall consist of the project manager or hisrepresentative, the safety officer, the site safety supervisor, the immediate person-in-charge of the worker or work involved in the accident or dangerous occurrenceand the sub-contractor's safety supervisor, if applicable

5.7 Records and Analysis

All information received and findings discovered from the investigation of anaccident or a dangerous occurrence shall be clearly entered in the incidentinvestigation records

Root cause of an accident or a dangerous occurrence shall be ascertained andpreventive counter measures shall be established and enforced to prevent arecurrence

Incident investigation records and photocopy of Notice of Accident I DangerousOccurrence shall be kept by the safety personnel in the relevant file at theworksite for the purposes of necessary reference and inspection by an inspector

5.9 Principles of IncidentI Accident Investigation

The 3 principles of accident investigation on site are gathering information,

analysing the facts and making recommendations

Gathering Information

When investigating an accident, the investigator should either the basic fact from questions based on

 Who (was the injured) and others involved in the accident?

 Where (did the accident happen)?

 When (did the accident happen)?

 How (did the accident happen)?

 What (were the contributing and immediate causes of the accident)?

 Why (analysis of accident)?

The investigator should also gather information from witness accounts byinterviewing those who were at the scene of accident and had witnessed it

When interview the witnesses:

 reassure them

 ask open-ended questions

 do not ask leading questions do not interrupt witnesses ask for any suggestionssummarize for the witness

 obtain written statements by interviewee

 Information can also be obtained by surveying the physical conditions of thesite When doing this, he will:

 Check the conditions and position of machinery/ equipment involved in theaccident ,assess the extent of damage , sketch the location of accident,position of machinery I equipment & prominent features and take relevantphotographs

 Existing records of statutory registrations, test certificates and inspection

checklist of machinery and I or equipment.

Analysing the Facts

6.0 In House HS&E Rules and Regulation

6.1 Objective

The objective of in-house safety rules and regulations is to ensure all personnelknow their obligations and responsibilities and comply with the rules andregulations

6.2 In House Rules and Regulation

 Provision and use of personal protective equipment

 Every person present in the construction area shall be equipped with basicPPE such as safety helmet, safety shoes safety harness etc at all times

 Every person works at a height of 2 meter or more shall be equipped with asafety harness with double lanyard at all times

 Every person exposed to the hazard of eye injury which might be caused either

by dust, airborne particles or any other flying object shall be equipped withsafety eye goggles or face-mask at all times

 Every person exposed to noise level of 85 dB (A) or above shall be equippedwith hearing protectors at all times

 Every person exposed to dusty or smoky working environment shall beequipped with respiratory protector at all times

 Every person exposed to the hazard of bodily contact with any corrosive, toxic,injurious or undesired substances shall be equipped with face-mask , handgloves, boots, overcoat, etc at all times

 All personal protective equipment provided shall be of the appropriate typesand approved by the Client or other relevant Authority

 All employees shall make full use of personal protective equipment issued tothem in order to protect their safety and health as well as to prevent theoccurrence of incident

 Special type of personal protective equipment might be required for certainspecialized construction activities and the Management shall decide on suchmatter

6.3 Standardization for the Wearing of Safety Helmets

All personnel at the worksite will wear safety helmets in accordance with the following colour code:

6.4 OperationI Maintenance of Machinery & Equipment

 All operators of plant, machinery and equipment shall possess appropriatequalification, experience and be approved, certified and licensed by theAuthority or other relevant Authority so as to meet with any of the applicablestatutory requirements

 All maintenance of plant, machinery and equipment shall be carried out bypeople who are qualified, approved, certified or licensed by the Authority orother relevant Authority so as to meet with any of the applicable statutoryrequirements

6.5 Handling, storage and use of materials

 Handling, storage and use of materials shall be in such manners as may berecommended by the manufacturer or supplier and if no such recommendation

is available; shall be in such generally acceptable manners so that the chances

of causing incident is minimized

 All materials shall be placed under the charge of appointed material controllerwho should be responsible for the receiving, storage and issuing of thematerials

 Person-in-charge who withdraws any material from the material controllershould be responsible for the safe use and disposal of such material

 Proper facility shall be provided for the safe storage of materials & Properfacility shall be provided for the safe holding of waste materials & for disposal

by licensed waste contractor

6.6 Reporting of hazards and incidents

 Any person who discovers any hazards or incidents should immediately

report such matters to his superior

 Any person who suffers any injury arising from his course of employmentshould notify his superior within the shortest period of time and in no caseshould be more than 3 days from the date he suffered the injury

6.7 Housekeeping

 Debris should be placed in waste bins of suitable size while waiting for disposal

by licensed waste collector Debris should not be indiscriminately dumpedaround the worksite as it might cause environmental pollution and breeding ofpest

 Food waste should be bagged and tied and placed in designated refuse binsand removed from the worksite daily by licensed waste collector

 Adequate and properly constructed sanitary facilities such as wash basins,toilets, bathrooms, etc should be provide d and maintained in good workingcondition at all times

 All employees shall make use of waste bins and sanitary facilities provided atthe worksite to maintain the housekeep ing of the worksite in a generallyacceptable condition

6.8 Removal of Fire Hazards

 All flammable substances such as diesel, kerosene, thinner, paint, etc shall bestored in well ventilated designated storage area with authorized access

 Unused flammable substances shall be returned to designated storage area atthe end of each working day Dirtied flammable substances such as diesel,kerosene, thinner, paint, etc shall also be returned to designated storage area

at the end of each working day for disposal by licensed waste collector

 Flammable solid waste such as paper, cardboard, plastic sheets, etc shall beremoved daily from construction area and placed in designated waste bins fordisposal by licensed waste collector

6.9 Enforcement of In-House Safety Rules and Regulations

 All trade person-in -charges shall have the responsibility to enforce the relevantin-house safety rules and regulations to ensure that all employees under theircharge comply with such rules and regulations and works are being carried out

in accordance with the appropriate manner and code of practice

 Stern disciplinary action shall be taken by the Management against thoseemployees who purposely or repeatedly violating safety rules and regulations

6.10 Training and Review of Rules and Regulations

The in-house safety rules and regulations shall be communicated to all levels ofthe organization through safety promotion, training and other means The in-house

6.10.1 Safety Sign

Warning signs at strategic points around the periphery of the Site where

trespassing is likely to occur Such signs shall have the words "DANGER - KEEP OUT" in the Two official languages painted in red on a white background

in gloss finishing paint Warning lights shall be placed at similar positions at night

to serve as warning

If work is carried out near public roads, all signage shall comply with the

recommendations of the Temporary Road Signage or as gazetted by the local government

6.10.2 Policy concerning repeat offenders

The following procedures shall be set up for repeated offenders on violating safety:

Verbal Warning: The offenders will first be given a verbal warning for any unsafe act by the PM /SS

Notification by letter: If the offender repeats the offence, he will be given a

notification of wrong doing by the PM / SS

 Fine: If the offender repeats the offence again, he will be fined by the PM / SS

 Removal from site: If the offender still persists with the unsafe act, he will

be removed from the site

6.11 Safety Rules and Regulation for all Site Staff & Workers

 Workers must fully use their safety personal protective equipment (PPE)

 Always check with your supervisor if you are doubtful

 Report injuries sustained to your immediate supervisor at once

 Safety Harness must be worn when working above 2 meters and must be properly anchored

 No throwing of objects or debris from height

 Strictly observe all safety/ warning signs posted & speed limit of Client site ( 10 KM/H)

 No littering - keep workplace clean Practice good housekeeping

 Report immediately of any unsafe act / condition to your supervisor

 Barricade all the opening at work area to prevent fall of person

 Never operate machines / equipment / explosive powered tools if you are not authorized

 Do not use defective tools / equipment Keep them properly after using

 Do not take short cuts - use proper access

 Never hitch a ride on suspended load or moving dumpers, excavators etc

 Any worker who violates safety rules and regulations will be severely dealt with

 Attend toolbox meetings and Participate actively in Safety Promotion

 Observe all safety measures and work safely at all times

 No discharge of waste water in to public drain /Sea

 Regular removal of construction waste & food waste

 Apply necessary permit prior to start works

8.2 Objective

The objective of evaluation, selection and control of sub-contractors is to ensurethat the subcontractors are aware of their obligations and that only sub-contractorswho can meet these obligations are employed at the worksite

8.3 Selection

Safety performance of sub-contractors in the past shall also be taken into account

in the selection of contractor In general, preference shall be given to contractor who shows improvement in the course of accident prevention along withsatisfactory work performance

The accident frequency rate and accident severity rate are two indicative statisticalfigures which could be used to compare the accident rate among various sub-contractors performing similar nature of work

8.5 Control

 Safety rules and regulations in force at the worksite shall be conveyed to contractor's top person-in-charge to ensure close & effective communication and observance

sub- Sub-contractor's safety supervisor shall possess pro-active attitude in safetysupervision work and shall attend as far as possible all safety committeemeetings and safety inspections and participate in all safety promotionalactivities being organized at the worksite

 They should report all reportable accidents and dangerous occurrences to theSafety Section

 They shall not misuse or damage any facility provided & they shall not create

an environment that could breed mosquitoes
