The Danang-Quang Ngai Expressway Project (DQEP) connects Danang city withQuang Ngai province through three (3) provinces and city including; Da Nang,Quang Nam, Quang Ngai Total length of the project is about 139.52km; ofwhich 131.50km is expressway and 8.02km is linking road
The starting point: Phu Hoa hamlet, Hoa Nhon commune, Hoa Vang district, DaNang city
The end point: La Ha 4 hamlet, Nghia Thuong commune, Tu Nghia district,Quang Ngai province

1.2 Site
The Site of the Contract Package A1 is from Km65+000 (ending point ofContract package 7) to Km81+150 (beginning point of Contract Package A2),total length of the package is of 16,150 meters The Site is located in Phu Ninhdistrict, Tam Ky city and Nui Thanh district of Quang Nam province

3 QUALITY ASSURANCE SYSTEM
Our Quality Target shall apply in this Project is :
“To produce good quality construction products consistent with the requirements of the Client as specified in the Contract”
Consequently, the Client’s needs and requirements as specified anddescribed in the contract will be carefully analyzed so that theserequirements are fully met by the proper selections of materials,workmanship, construction techniques and the safety of work toachieve the required quality construction product
The management will set up Project Quality Control System toimplement and verify compliance with the necessary qualityrequirements Periodic and regular audits will be conducted toreview and update the status and adequacy of the Quality ControlSystem
Every person assigned to this project is expected to contribute tothe achievement of the quality construction product

3.1.3 Responsibility and Authority

of the Project lies with the Project Manager, an effective ProjectQuality Assurance System is the product of all employees on theProject which are involved in the development, review andimplementation of systems, processes, procedures and workpractices, particularly those personnel who have the authority to:(a) initiate action to prevent occurrence of a product non-conformity;
(b) identify and record any product quality problems;
(c) initiate, recommend and provide solutions through suitablechannels;
(d) verify the implementation of solution;
control further processing, installation or delivery of non-conformingproduct till correction is made good
The Project Manager shall be responsible for themanagement of the Project as guided Target decision of theBidder He has the authority and responsibility forengineering, scheduling, cost, industrial relations andcontract administration of the Project
His responsibilities in respect of quality include:
QA/QC Plans and ensuring that all activities relating toquality are performed in compliance with the requirements

c) Deputy Project Manager No2: (Project Quality AssuranceManager)
The Deputy Project Manager No2 shall assist/support theProject Manager’s duties described above and mainly are on
fields: Technical/Economic & Planing/Project Quality
Assurance/Environmental/Health & Safety Manager.
He is the nominated quality representative for the Project
He has the responsibility and authority to ensure theimplementation, maintenance and control of the ProjectQuality Assurance Plan and reports to the Manager, QualityAssurance / the Project Manager on the performance of theQuality System for review and as a basis for improvement ofthe Quality System He is also responsible for orderingpermanent material and controls their quality to becomplying with the project specification and client’srequirement all the time
d) Deputy Project Manager No3:
The Deputy Project Manager shall assist/support the ProjectManager’s duties described above and mainly are on fields:Accounting and Financial Manager/Administration
The Project Quality Engineers are responsible forestablishing, implementing and maintenance ofdocumented procedures and inspection and test plans Theyshall ensure that inspection and testing activities arecontrolled in order to verify that technical and specifiedrequirements are met
Trang 7They report to the Project Quality Assurance Manager andinteracts with other Managers and Engineers to review,implement and manage the quality requirements and thequality plan.
They shall interface with project suppliers including contractors to ensure materials, products and equipmentsare inspected, tested measured and calibrated or otherwiseverified to demonstrate conformance to specifiedrequirements and the quality plan
sub-They shall maintain, compile and review applicabledocuments and quality records to ensure efficient retrievaland/or are available at all locations where operationsessential to the effective functioning of the quality systemare performed
Quantity surveyor is responsible for the effective projectcontrol activities for the Project He report to the ProjectManager/Deputy Project Manager No2 and responsible forthe planning, cost engineering, cost control planning andscheduling functions for the Project including:
scheduling and cost as required by Project Manager;
project for compilation of information regarding mainactivity areas for monthly reporting;
overall programs;
coordinates the work scheduling and procurement
The Construction Manager of each construction sectionreports to Project Manager/Deputy Project Manager No1 Hehas the responsibility and authority to :
nonconformities relating to the product, process andthe Quality System ;

He is responsible for assisting the Engineer’s duties and tocontrol the subcontractor or construction teams in respect ofquality assurance as directed by the Engineer
He is responsible for maintenance and arrangement ofmachineries, which will be used for the site works every day
in order to carry out the works efficiently and appropriately
as directed by the Construction Manager
He is responsible for all the permanent material tests to becarried out at site laboratory with laboratory staff andkeeping those test record in the file appropriately He shallidentify whether the tested material is comply to thecontract
In case any material fail the test, he shall report toconstruction manager and engineers then take appropriateaction to non-conformance products/potential non-conformity
He/She is responsible for controlling the revision and status
of working drawings under the direction of Project Engineer

communication facilities, emergency and first aidprovisions;

3.2 Quality Assurence System
3.2.1 Structure of Documentation
The Quality Assurence System of this Project consists of the QualityManuals of the Bidder and the Project Quality Plans They areapplicable to the operation of various system activities and aredescribed in the relevant appendixes and Cross Reference to ISO9001-2000 The scope is governed from the review of the projectcontract conditions and specifications
3.2.2 Project Quality Assurance Plan ( PQAP)
The Project Quality Assurance Plan is in accordance to therequirements of ISO 9001:2000, and is jointly developed by the headoffice Quality Assurance Manager and the Project Manager togetherwith the Project Quality Assurance Manager and Engineers and shall
be endorsed by the head office Quality Assurance Manager
The Project Quality Assurance Plan shall be subject to continuousreview and modified as required by the Project Manager / ProjectQuality Assurance Manager and Project Quality Engineers
Checking shall be carried out by the relevant Construction Manager.The Project Manager may endorse quality plan for immediateimplementation prior to endorsement by the head office QualityAssurance Manager
3.2.3 Project Quality Control Plan ( PQCP )
The Project Quality Control Plan shall define the processes,procedures and controls for the engineering and technical aspects onthe execution of the Project and describe Where, When, Who, Whatand How the activity shall be performed
Trang 11General procedures shall follow those laid down in the QualityControl Manual and the specific sequence of activities and qualitypractices relevant to this Project shall be jointly developed by theProject Quality Engineer and the person in charge of each section.

The format of the Project Quality Control Plan is as follows

Define and describe the procedures in the following order
Set out receiving, storage and inspection procedure of material
Set out work procedure, activities and instructions in sequence
Set out procedures for handover a part or whole of work

To specify acceptability for all features and requirements

To define the method, frequency and the inspector who has theresponsibility and authority to verify activities

While the Head office Quality Assurance Manager is responsiblefor review and re-issue of the Quality Manual and the QualityAssurance Manual, the Project Quality Assurance Manager isresponsible to maintain and remove obsolete pages or sections
of the Document whenever revised copy issued
(1) Project Quality Assurance Plan, Project Quality Control Plan
Changes shall be made jointly by the Project QualityAssurance Manager and the person in charge of eachsection with approval from the Project Manager
> Refer to clauses 3.2.2 "Project Quality Assurance Plan" and 3.2.3 "Project Quality Control Plan"
Changes shall be approved by the Project Manager andsigned on the cover page
Revision No., Date and Description shall be shown on thecover page
The summary sheet shall be used for identification of thenature of the changes
Distribution lists shall be prepared, kept and maintained bythe Project Quality Assurance Manager The control Copy of
document shall be identified as controlled by as stamped
before issue The Project Quality Assurance Manager shallensure that the correct documents together with therespective form used throughout the period of the projectare used
(2) Contract and Project Execution Documents
Contract documents are defined as all documents anddrawings which form part of the Contract such as Conditions
of Contract, Tender List Submission, Specifications, Bills ofQuantities with prices, Engineer’s Instructions, Variations,Drawings, Method Statements, Schedule of Rates, EquipmentList and Work Schedules for the Project
Trang 13Project execution documents shall be all documents /drawings submitted or received during execution of theContract.
Further to the control measures, the following will beincorporated:
indicating the file location and the person in-charge
process will be established to link and cross referencerelated correspondence to enable fast search and easyretrieval
then stored in a secured storage room to preventdeterioration and damage
A comprehensive document / drawing control processencompassing submission, transmittal maintenance anddistribution shall be documented in compliance with theClient’s requirements
The Project Quality Assurance Manager is responsible to provide,maintain and make the latest or specific version of the standardsand codes of practice available to the staffs
(5) Correspondences
Correspondences relevant to the Quality such as the Customer'sWork Order/Instruction, Request/Complaint, to and from Sub-Contractor or Vendor shall be registered, inspected or approved andcirculated Details of procedure are defined in the Project QualityControl Plan
a) Inspection / Approval
The Project Manager has the responsibility and authority toinspect the incoming correspondence and approve/sign theoutgoing correspondence
> Refer to clause 3.6.1 "Contract Review" for the incoming correspondence from the Customer
b) Registration / Filing
The appointed person normally the receptionist/typist isresponsible to register and file the correspondence Thereceptionist / typist shall keep and maintain the register andmake correspondence available and accessible to the staffs.c) Distribution / Circulation
