Scholarship Application Form

Instruction Sheet 2019-2020

1 Students must meet all scholarship requirements listed below:

 Student must have a cumulative GPA of 3.0 or above.

 Student must be currently enrolled as a second through fifth year student with at least one full school year remaining until graduation

 Student must be full-time status (minimum 12 hours) in an accredited college or

university for both fall and spring semesters of the coming year Scholarships are for one year

 The student must be pursuing a degree in a STEM (science, technology, engineering, or math) major

2 Read the application form carefully and try to respond fully to all sections Responses on the

application must be typed

3 Print and sign the application and mail the completed application to the following address: (Do not hand deliver or email, applications must be post marked by May 31, 2019.)

4 Transcripts must be official copies issued by the academic institution If the transcripts are not

mailed separately, but are attached to the application, they must remain in the original, sealed envelope from the institution’s registrar’s office

5 Letters of recommendation may be sent with the application or independently mailed to the

address above These may not come from family members and need to be signed and sealed.

Essay

Answer the following essay to the best of your ability and attach to the application:

Explain how Petroleum based products impact your day to day life Be sure to include specific examples and try to think beyond the obvious answers, such as fuel for a vehicle.

Note: The essay is an important part of this application, and should be given serious thought by the applicant There is not a length

requirement, but a target of 300-450 total words for each essay is recommended
