The primary purpose of the Person County Health Department is to provide
public health services that allow the promotion of optimal health and well
being of the citizens of Person County The focus of service is health
promotion and disease prevention
B PRIMARY PURPOSE OF POSITION:
Participates as a member of the health care team providing direct patient care
to the adults, maternity, family planning , and STD
Works in conjunction with program coordinators in all programs offered at the Person County Health Department
C WORK SCHEDULE:
Regular work hours are Monday through Friday, 8:00 am to 4:30 pm with
one hour for lunch Assignments could include after hours and/or weekends
on occasion A flex schedule may be utilized upon agreement with the PHN

II A DESCRIPTION OF RESPONSIBILITIES AND DUTIES:
1 Participates in Public Health Clinic Services 5 days a week Performs a complete physical examination on both pregnant and non-pregnant women including all body systems; identifies any abnormalities and clearly documents findings
2 Orders, interprets, manages routine and non-routine laboratory tests; recognizes implications of abnormal lab results, and orders appropriate plan of care
3 Diagnoses medical conditions, formulates plan of care, and
appropriately prescribes, and/or refers for patients’ total health care
4 Manages all methods of contraception including emergency
contraception; performs a wide variety of medical acts and renders emergency care if need arises (including CPR, treatment of shock, treatment of anaphylaxis)
5 Manages complete ante partum care of pregnant women independently and renders emergency care if need arises (including CPR, fetal
delivery, treatment of shock, treatment of anaphylaxis)
6 Teaches patients and their families about their health needs and how to care for themselves to achieve improved health outcomes
7 Works in conjunction with Maternity Program Coordinator, and serves
as back-up, with maternity patient’s plan of care and changes as needed Schedules and case manages referrals for ultrasound, GTT, Rhogam Injection and 17P injection Monitors and follow-up of patients and plan of care as deemed necessary Provides maternal expanded role by assessing, instructing and physician communication for the maternity patient and family Documents accurately and in a timely manner according to agency protocol
8 Coordinates care plan with the Pregnancy Care Management Program
as needed Works closely with Pregnancy Care Management in developing and follow through with care planning in the Pregnancy Care Management Program
Trang 39 Assists in carrying out care plan of care developed by appropriate Program Coordinators Prepares requisitions for lab work, updates documents on patient records, whether paper or electronic records
10 Assists in follow-up of clients who have had referrals to other Health Care Providers Provides direct patient care and assistance in all clinics
as needed Takes histories, teaches patients/parents to understand cause, effect and control of health care Assists medical providers as needed when caring for patient
11 Administers Depo injections, preventive immunizations and antibiotic injections in Family Planning, Maternity, Adult Health, and STD Clinics Administers TB skin tests and accurately reads results
Documents appropriately in NCEDDS and in EMR in accordance with policy
12 Reviews, revises and writes program policies
13 Collaborates with Health Education/Promotion in developing individual and community programs ex: Incentive Grants, Health Maintenance Programs Works in conjunction with Health Educator at events as needed
14 Teaches patients to maintain a state of wellness through regular pap exams, breast exams, mammograms, health maintenance exams, and programs Provides nutritional counseling and education
15 Performs Newborn Visits and Postpartum Visits as needed according to Program guidelines
16 Completes daily logs, travel, time sheets, request for leave forms and time studies in a timely manner
17 Serves on various in-house committees as deemed appropriate by PHN Supervisor and/or Health Director

EXPERIENCE REQUIREMENTS:
A Knowledge, Skills and Abilities:
Registered Nurse with public health education/experience with knowledge of and skill in the applications of nursing theory, practices and principles of public health
B 1 Required Minimum Training:
Graduation from a Physician Assistant program approved by the N.C Board of Medical Examiners and preferably one year of experience as a Physician Assistant; OR licensed as a Registered Nurse by the NC Board
of Nursing, graduation from a Nurse Practitioner program approved by the N.C Joint Subcommittee of the Board of Nursing and Board of Medical Examiners, or graduation from a Master’s Degree in Nursing with concentration in Nurse Midwifery Program with one year experience as a Midwife National certification as a Physician Assistant, Nurse
Trang 7Practitioner, or Nurse Midwife is required Must have a reliable means of transportation for job- related travel
