Job Title: Specialty Doctor in Perinatal Psychiatry
Responsible to: Consultants- Dr Rina Gupta, Dr Daniela Patrascu

Location: Nominal base is Linden Centre but required to work at
other Trust locations as requested in response to the demands of the service

The Role of the Specialty Doctor in Psychiatry
The specialty doctor will be expected to work independently and provide clinical leadership to the team, through good multi-professional working The specialty doctor will have shared responsibility for the clinical care of those patients referred to the team and will be expected to work closely with the consultant and team manager to ensure the provision of an effective service
Duties and Responsibilities of all Specialty Doctors
1 Observe the Trust’s agreed policies and procedures
2 Provide, with consultant colleagues, a service in the specialty for the Trust with
responsibility for the prevention, diagnosis and treatment of illness, and the proper functioning of the department
3 Honour out-of-hours responsibilities, including rota commitments where
contracted
4 Provide agreed cover for colleague’s periods of leave
5 Participate in audit and continuing medical education
6 Contribute to the training of junior medical staff and medical students
7 Be involved in research and teaching where appropriate

Working independently and accountably as part of a team of clinicians offering Mental Health services Working collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7 day a week, 24 hours a day service for 365 days a year
In delivering your duties you are expected to display behaviours that are aligned with the Trust values and as required in accordance with the GMCs Good Medical Practice at all times
The Community Perinatal Services for Essex provides multidisciplinary, community specialist care for the assessment and treatment of woman with mental health
illnesses in pregnancy and postnatally across the County of Essex The post- holder will serve the needs of patients across the North of Essex, which includes the areas linked to the three main hospitals, which are Princess Alexandria in Harlow,
Colchester General and Broomfield Hospital This will be reviewed periodically to ensure appropriate resource allocation to the different areas and any changes made shall be after deliberation
The total birth rate for the population covered by the service is around 23,000 live births per annum The service works flexibly to meet the needs of the patients and their families The patients are supported wherever it is most convenient for them; at home, in local surgeries, maternity hospitals, clinics or children centres Though the service does not do crisis intervention yet it does contribute to both supporting them
in times of emergency during working hours Other services include liaison work the maternity services
Clinical Responsibilities
1 Provide high quality medical care with support from consultant perinatal
psychiatrists across the Perinatal Service
2 Maintain the high level of expertise and skill necessary for the provision of
excellent quality perinatal mental health care, aiming for optimal outcomes for mother, infant and the wider family
3 Provide medical input to the Perinatal Service’s patients in a community setting in
line with the agreed job plan This includes assessments of women’s mental health and mother infant relationships and related risks, bio-psycho-social diagnostic formulations, exploration of prevention, treatment and care options, clear communication and shared decision making with patients (and families
as appropriate), communication and joint planning with other professionals and agencies by the most effective means, use of appropriate legal statutes
Trang 4and powers for example the MHA, MCA AND Children Act where necessary for good patient care
4 Ensure that care provided is in line with vision and values of the service for patient
centred, compassionate, high quality care focussed on the needs of the woman, the baby and the wider family, facilitating shared decision making at all times
5 Ensure highest standards of transparency and clarity in communication with
women and other professionals (e.g all correspondence addressed to the patient and copied to all professionals)
6 Support the Perinatal Consultant Psychiatrists in providing clinical supervision to
junior doctors that are working or training in the service, and offering clinical advice and information as and when required to all staff
7 Work closely with all professionals in the Service and with other agencies,
including voluntary organisations to ensure that high quality patient care and after-care is effectively co-ordinated
8 Actively contribute to quality improvement activities in the Service This includes
participating in, and supporting or supervising others in initiating and completing projects, including audits, quality improvement initiatives, service evaluation and research This should include both local and regional or national projects
1 Participate in continuous evaluations/audits of the Service, including national
audits and benchmarking These include RCPsych CCQI Perinatal Network Peer reviews and related surveys, CCQI visits, and reports to commissioners Actively engage in quality improvement responses to any recommendations from these or as may emerge from time to time from other sources (eg Confidential Enquiries; NICE Guidelines)
2 Participate in service evaluation and the planning of future service developments
3 Ensure that cover is arranged for responsibilities during leave with consultant
colleagues within the service
TRAINING:
1 Participate in educational supervision to any future trainees
2 Participate in undergraduate and postgraduate medical teaching
3 Participate in the training of other disciplines and groups inside and outside
the service This includes:
• Challenge stigma and discrimination against pregnant and postnatal women with mental disorder, by teaching and raising awareness with the public, public representatives, professionals and patients
Trang 5• Design and deliver training packages on awareness, prevention and detection of perinatal mental health disorders to meet the needs of local maternity, primary care, Social Services and other psychiatric colleagues
• Enhance public mental health through physical health promotion in pregnancy, and education for patients and professional groups on transgenerational effects of poor maternal mental health
4 Take part in continuing professional development, appraisal and revalidation
activities
Team
The perinatal service in Essex has 2 main hubs, North Essex and South Essex This
is further divided into 5 smaller hubs linked around the 5 acute hospitals The
speciality doctor will be working in the North Essex area The whole service has one service manager (8b), 2 clinical managers (8a) and 3.8 WTE Consultants The
service is rapidly expanding so there will be changes in the team members over time The service
• Provides assessment, treatment and management of moderate –
severe perinatal mental health disorders and high risk service users who have a GP in Essex, or who are planning to deliver their babies
in Essex, during the perinatal period (antenatal and for up to 24 months post-natally
• Provides support to mothers on leave or recently discharged from the Rainbow Mother and Baby Unit (MBU) and other MBU’s in order to facilitate early discharge and prevent relapses
• Provides preconception counselling
• Provides consultation liaison with GPs, antenatal services, midwives and health visitors to support women being managed in primary care and prevent deterioration
• Works in partnership with other mental health teams providing support
to women with perinatal mental health disorders (Mental Health Liaison Teams, Crisis Teams, CMHTs, IAPT) and with other services in contact with women (Antenatal services; Primary Care; Health Visitors;
Children’s Social Services)
• Delivers specialist training for other mental health teams, midwives, GPs and health visitors who will work with women with perinatal mental health problems
• Provides information for pregnant women and their partners and families about perinatal mental illness
• Supports partners of the un-well mother and can provide initial
assessments of their mental health and appropriate signposting
• Operates across the county

Resources
The post holder will have the use of PC and have access to the doctor’s rooms and the appropriate secretarial/administration support provided by a team The
community perinatal services uses the electronic record system – SYSTEM 1 but the post holder will also be expected to use other systems used in the trust for example PARIS and MOBIUS
Training, Research and Continuing Professional Development
The post holder is expected to participate in continuing professional development Study leave and facilities to enable the post holder to do this, up to the standard required by the Royal College of Psychiatrists, will be provided
Educational Activities of the Trust
The Trust has a policy of encouraging continuing professional development and will meet the Royal College’s requirements for continuing professional development by allowing the post holder appropriate paid study leave and reimbursing reasonable expenses
There is a rotational psychiatric training scheme for trainees, which have the full approval of the Royal College of Psychiatrists There is a comprehensive teaching programme consisting of case presentations, journal clubs, seminars, training in interviewing skills and lectures
The Trust has an active Clinical Audit programme with presentations of audits arranged on a three monthly basis
The Trust is committed to a policy of further education and training for all disciplines This includes links with Essex University - MA Courses in Psychotherapy, Research
& Management, Health & Sociology and Community Mental Health are run jointly with the Trust
There are strong links with the Colchester Institute, the University of Essex and Anglia Ruskin University, which run a number of degree programmes to which Trust

Appraisal & Revalidation
The postholder must actively participate in professional, clinical and managerial supervision and undertake relevant training aligned with the post holders personal development plan and other relevant professional bodies guidelines regarding CPD
The Trust is committed to medical revalidation and appraisal and the post holder is required to undergo annual appraisals to support the revalidation process and
continuous improvement of services The appraisal process is in accordance with the Trust’s Medical Appraisal Policy The trust uses an electronic appraisal system (Clarity) to support revalidation and collate Patient and Colleage 360 Feedback The Responsible Officer for the Trust is the Medical Director
Clinical Audit
Medical and clinical audit committees meet regularly and there would be
opportunities to attend appropriate audit groups There will be a requirement in the locality to ensure NICE guidance is appropriately addressed and followed

A job plan will be agreed upon with the successful candidate in conjunction with their supervising consultant and the Clinical Director The job plan will be reviewed on an annual basis or more frequently if changes are required due to service

Annual Leave
The post holder’s annual leave will be calculated pro-rata in line with NHS experience per year Absence during annual leave period will be covered by the colleagues
Study Leave
