Croup
■ Essentials of Diagnosis
• Viral croup—inflammation of the larynx and upper airway tures causes sudden-onset barking cough, stridor, congestion,
struc-± fever Usually self-limited Common in children <5 years
• Viral organisms causing croup—parainfluenza, respiratory
syn-cytial virus (RSV), influenza, rubeola, adenovirus, Mycoplasma
• Laryngeal cleft—failure of posterior cricoid fusion causes stridorand recurrent aspiration
• Angioedema—histamine-mediated acute swelling of skin and/ormultiple other systems may involve the larynx and trachea Caused
by allergy, C1 esterase deficiency, medications converting enzyme [ACE] inhibitors), and cold exposure
(angiotensin-• Laryngeal/esophageal foreign body
• Retropharyngeal abscess or tumor, mediastinal tumor
• Subglottic stenosis—congenital or secondary to endotrachealintubation
• Most viral croup is self-limited Stridor usually lasts 3–5 days
• Use humidified air especially at night Maintain good hydrationand food intake
• Stridor at rest or hypoxia should be treated with oxygen; inhalenebulized racemic epinephrine or budesonide
• Dexamethasone (single dose 6 mg/kg intramuscularly [IM] or.15 mg/kg orally) improves symptoms and reduces frequency ofintubation
• Severe dyspnea/hypoxia, exhaustion, and respiratory failurerequire endotracheal intubation
The younger the child, the smaller diameter of the trachea, the higher the chance that swelling of the airway caused by viral infection may produce respiratory obstruction Monitor the young child with croup carefully!
