SCHOLARSHIP PROGRAM

Good Hands Home Care is a recognized non-medical in home care agency serving throughout the mid-south Coming from disadvantaged backgrounds, the owners tremendously benefited from alumni and local scholarships to help fund their college education In turn, allowing them an

opportunity to pursue their dreams The Good Hands Scholarship Program offers awards up to

$400, non-renewable scholarship that recognizes graduating high school seniors for academic

achievement It provides financial assistance to help deserving students accomplish their future educational goals Good Hands Home Care does this in recognition of the continuing need to promote higher education for future generations

 Applicant must be a graduating high school senior in 2016

 Applicant must reside in the following TN counties – Shelby, Fayette and Tipton

 Applicant must attend an institution of higher learning

 Applicant must have an acceptance letter from an institution of higher learning

 Applicant must have a financial need

 Applicant must have an academic grade point average of 2.5 or higher

 Applicant must have provided service to their community

 Application must be submitted on the current Scholarship Form Incomplete

applications or submissions that do not follow the submission criteria will not be

processed or considered for evaluation

 Applicant must include two letters of recommendation from: (1) high school

principal, academic dean, or guidance counselor and (2) a high school teacher

Applicant’s scholastic achievements, leadership ability, extracurricular activities,

career/academic goals and financial need will be considered and must be documented in

letters of recommendation and summarized on the form

AWARD SELECTION PROCEDURE

A selection panel will review all applications received and make final recommendations to the Good Hands Executive Board, who will approve the final award winners Awardees will be notified in May 2016 if they have been awarded a scholarship No phone calls please

In May 2016, award checks up to $400.00 will be issued at an award breakfast presentations Pictures and names of recipients may be publicized at Good Hands website

to be considered by the selection panel No exceptions to this deadline will be allowed Any applications received
