The client must obtain the application form from the Companion Card website;

If they are able, clients are able to complete the form themselves or with the assistance of another person (friend, family member, service provider etc.)
Items 1-7 are personal details
The client must also provide two passport sized (45mm x 35 mm) photos, they must be identical,
colour, of the head and shoulders only and printed on photo paper
It must be signed by the client, their legal guardian or their agent if they have one
Items 1-8 and 11 must be filled out.
Item 8 if the applicant receives any of the services or supports outlined in item 9; the manager of the service completes item 9 If they do not receive any of those outlined in item 9; their health professional must complete item 10
Program managers are the relevant person to fill out item 9
Health professionals for item 10 can be; doctors, registered nurses, physiotherapists, psychologist, occupational therapist, social worker or speech pathologist
