• In case of power failure, turn off all switches immediately If not, equipment will start accidentally will start accidentally when the power is restored, which may cause an injury

• Neglecting these abnormal circumstances of machine equipment, temporary machines and materials, or installation equipment leads to the occurrence of fatal accidents or accidents

• Report “ When, where, who, why, what”.
• Specify the emergency contact number (installation section) in Mitsubishi Electric Corporation, MELTEC, and subcontractor on the safety documents stored on site and safety control table posted on site.

(2) Rescue give first aid

Ask neighbors for help to rescue a victim.
Give first aid calmly and quickly according to proper order.
Request of a private car or an ambulance shall be done by the chief safety & health officer or an office staff on site.
If a private car is always parked on site, send a victim to predetermined hospital by that car immediately, not just waiting for an ambulance.
In this case, contact the hospital before leaving the site.
(3) Call an ambulance Call an ambulance when a site representative decides to urgently transfer a victim to the hospital,
based on the situation of fatal accident and the condition of victim.
The following shall be reported:
a Site name and its address.
b The situation of fatal accident and the condition of victim.
(e.g.): “ A worker hit his waist and cannot move We request an ambulance, please.”
Notes:
- Avoid to say “he is dead” or “he is dying”, which is just an assumption of amateurs An ambulance does not come for a dead person, and above all, death can be judged only by a doctor.
-Decide who will get in an ambulance with a victim.
-An attendant to a victim will give his information to the site office from the hospital and vice versa -Bring a copy of worker list to that an attendant can tell victim’s address, name, and age.
- Prepare information about the time of accident, how the fatal accident occurs, and the first aid on site
to report.

4) Send a victim to the hospital
and contact Carry a victim into an ambulance with an attendant as soon as it arrives. When a victim’s address, name, age, time of fatal accident, and his working contents are asked
at the hospital, an attendant should tell the same thing as told to the emergency services.
The hospital staff will ask whether a victim uses industrial injury insurance or not When the accident clearly belongs to an industrial accident tell a fact.
An attendant should disclose own identity, and tell that a person in charge well come to the hospital from the company The person in charge needs to confirm a industrial injury insurance number in advance.
Right after completing a procedure, an attendant should call the site representative and the company to tell which hospital the victim is in.
An attendant should ask doctors and nurses about the injury of a victim Tell it to the site representative and the company (Every 30 minutes)
When a doctor asks an attendant to call a victim’s family he should call the site representative to contact the company in order to call the family.
5) Contact persons involved A fatal accident should be reported to persons involved at least before arriving an ambulance.
Make sure to inform all persons involved However, tell them that the company reports the fatal accident to the family if necessary.
-Prepare a memo in advance and tell them about the following:
a- Time of fatal accident b- Place.
c- Condition.
d- Victim’s address, name, age, and job description.

-The safety belt must restrict a fall to 1.5m or less
-When working on site using scaffolding method, wear two safety belts
When there is a danger of falling at 2m or higher places to the working level, all workers must wear them when working at a place with danger of falling even less than 2m to the working level.

Trang 26POINTS TO PREVENT ELECTRIC FATAL ACCIDENTS
• 1 Install ground-fault interrupter to prevent an electric shock
• 2 Prohibit wiring which has contac with steel beams, reinforcing bars, and steel pipe scaffoldings, etc.
• 3 Prohibit wiring on the ground.
• 4 Replace broken switches (cover, etc).
• 5 Install the earth completely.
• 6 Waterproof measures for outdoor switch.
• 7 Waterproof measures for outdoor equipment
• 8.Maintain proper wiring height.
• 9 Tape connecting and terminal parts.
• 10 Install a fence for high voltage facilities, indicate “Keep out” and “Danger” sign, and assign the person in charge.
• 11 Inspect carry-in equipment before entering the site (Avoid carrying of any defective equipment.)
• 12 Remove unnecessary wiring.

13 items for the installation by WOS methos
• 1-Install safety barricades blocking the hoistway opening to prevent any objects from flying or falling from the entrance opening.
• 2-Install a protective cover to traction sheave and governor in machine room.
• 3-Install a 90cm or more high safety handrail with intermediate handrail bar and kick plate to the upper/lower work platforms and install a removable gateway to the area facing to the hall.
• 4-Install the ceiling to prevent any objects flying to upper work platform.
• 5-Install a wire netting or curtain to the area facing to the CWT in the upper/lower work platforms to prevent the contact with CWT.
• 6-Install a red warning lamp in the machine room to indicate that the power for elevator operation in on.
• 7-Install an alarm buzzer in the machine room and car to inform the car operation.
• 8-Install a key switch for the operation so that other workers cannot operation the car without permission.
• 9-Use wire ropes which are cut into the predetermined length and inserted into the rope socket at both ends.
• 10-Install a stopper on the CWT to prevent the CWT block from deflecting from the CWT guide rail.
• 11-To install rail brackets in order with moving the gondola upward, install a contact segment for the overrun stop
switch the the guide rail to prevent the bottom guide shoe from moving upward over the installation position of rail bracket is set.
• 12-Install a nameplate to safety handrail in which the cautions such as maximum load, and the prohibition of work in upper/lower worker platforms at the same time are written.
• 13- A chief installer using this method shall be the trained worker who participates in the classroom lecture and practical skill education held by the elevator manufacturer, and shall supervise and administrate the work
