Elementary School Application Purpose

The purpose of the Outstanding Achievement Award, created in honor of Saint Catherine of Siena, is to recognize and support qualifying students completing Elementary School

Requirements

Every applicant’s family must be registered at St Catherine of Siena Parish Applicants must

submit the application form, evidence of parish membership (This is your family Parishioner ID Number), three reference forms (completed by teachers or counselor or other school official), and

a personal essay The following are the areas the requirements for consideration:

Elementary School Fifth Grade Students:

- Completed the 2019-2020 school year

- Promoted to Middle School (Junior High school)

- Have high academic record

- Participated in extra-curricular activities

- Volunteered in church/community related activities

Application Materials:

Candidate must submit the following:

• Completed Application form

• Personal essay about individual circumstances and why the student is a promising candidate for the award

• Three complete recommendation forms (At least one should be from the student’s

teacher The other two could be from a counselor, coach or other adult who is familiar with the student’s extracurricular or volunteer activities)

• Evidence of St Catherine Parish membership

• School transcripts/report card

• Other evidence of requirements

DEADLINE to submit complete application packet to the Parish main office is:

April 24, 2020 at 5:00 p.m
