(External assistance)

Nội dung:

- Chỉ dẫn y tế qua Radio

- Vận chuyển bệnh nhân bằng trực thăng

- Vấn đề liên lạc với bác sĩ

I Chỉ dẫn y tế qua radio ( Radio Medical Advice):

* Chỉ dẫn y tế qua Radio được thực hiện bằng các phương tiện điện tín hay điện thoại trực tiếp với bác sĩ của 1 số cảng thuộc tấc cả các khu vực trên thế giới

* Cũng có thể nhận được sự chỉ dẫn của một tàu khác gần đó đang có bác sĩ trên tàu

* Tốt nhất việc trao đổi thông tin theo 1 ngôn ngữ chung

* Trước khi trao đổi với bác sĩ cần chuẩn bị sẵn:

- Giấy bút để ghi chép và nhớ sao lại những ghi chép này vào hồ sơ tàu và bệnh

án bệnh nhân

- Có thể ghi lại những trao đổi thông tin vào băng casset

- Chuẩn bị sẵn danh mục thuốc bên cạnh

- Chuẩn bị sẵn mẫu câu: + Phần A: Cho trường hợp ốm đau

+ Phần B: Cho trường hợp bị thương

II Vận chuyển bệnh nhân bằng trực thăng ( Evacution by helicopter):

* Chỉ yêu cầu trực thăng trong tình huống nghiêm trọng

* Thủ tục tiến hành:

- Liên lạc trạm Radio bờ xin chỉ dẫn

- Chuyển đến bác sĩ tất cả những thông tin về bệnh nhân → bác sĩ cho chỉ dẫn chăm sóc tức thì

- Báo cho cơ quan biên phòng bờ biển về phương pháp vận chuyển bằng trực thăng

- Cơ quan biên phòng sẽ chuẩn bị và giữu liên lạc với tàu

* Đừng mong trực thăng xuất hiện tức thì

* Khi vận chuyển bằng trực thăng đã được quyết định những việc sau đây phải được tiến hành:

1 Báo: - Vị trí tàu

- Loại tàu

- Màu sắc thân tàu

2 Báo chi tiết về tình trạng BN và yêu cầu về vận chuyển nạn nhân (VD phải được vận chuyển bằng cáng)

3 Cử người trực liên lạc với trực thăng

4 Trực thăng thường trang bị truyền tin Radio loại VHF hay UHF không thể liên lạc trên tần số MF

Nhưng một số trực thăng lớn có thể liên lạc trên tần số 2182 KHZ MF

5 Tàu phải hành trình không đổi và tốc độ tối thiểu

6 Thông báo cho tổ lái hướng gió

7 Chuẩn bị 1 khu vực rộng trên mặt boong hoặc trên nắp hầm hàng và vẽ chữ "H" thật to màu trắng Tất cả dây, cáp, ăngten chung quanh đó phải di chuyển

Nếu tất cả sẵn sàng thì trực thăng hạ cánh, hoặc thả thang dây hay thả cáng

8 Phải đảm bảo an toàn cho hạ cánh và cất cánh Gió tạt xuống từ cánh quạt trực thăng sẽ làm hất tung vải nhựa, dây cao su, dây thừng, … có thể hút vào động cơ gây nguy hiểm cho máy bay

9 Ở vùng biển có nhiều tàu cùng hoạt động -> tổ lái khó xác định mục tiêu tìm kiếm Vì vậy cần dùng tín hiệu để tổ lái dễ phát hiện như:

• Tín hiệu khói màu da cam

• Tín hiệu đèn Aldis

10 Không bao giờ móc thang dây của trực thăng vào tàu

11 Việc treo dây tời chỉ cho phép những người đi găng cao su (Trực thăng có thể tích nạp tĩnh điện có thể gây nguy hiểm)

12 Bệnh nhân được đặt trong cáng Neil - Robertson thì có thể đưa thẳng vào trực thăng hoặc đặt trong cáng cứng

13 Toàn bộ thời gian này phải tuân thủ mọi sự chỉ dẫn của phi hành đoàn

14 Nếu cứu nạn vào ban đêm thì phải dọi đèn pha theo hướng thẳng đứng để định vị tàu - Không được chiếu đèn lên máy bay

Chuẩn bị để chuyển bệnh nhân ( Preparation of the patient for evacuation)

* Bỏ vào túi plastic

• Hồ sơ

• Bệnh án (nếu có)

• Các giấy tờ cần thiết khác

• Ghi thêm các biện pháp điều trị đã tiến hành

* Nếu đã dùng Mocphine -> treo 1 thẻ vào người nạn nhân

* Mặc thêm áo cứu sinh nếu cần

III Vấn đề liên lạc với bác sĩ ( Communicating with doctors):

Với tác phong lịch sự, khi gửi bệnh nhân tới bác sĩ cần thiết viết 1 thư với những thông tin đầy đủ về bệnh nhân:

• Họ tên thuyền viên

• Ngày sinh

• Tên tàu

• Tên đại lý

• Chủ tàu

Các nội dung về y tế: Trình bày tóm tắt, hệ thống bản sao về thông tin các bác sĩ ở các tàu, các cảng trước theo mẫu phần sau:

FORM FOR CASE REPORTING, REFERAL, EVACUATION

Information about a seafarer's injury or illness should be recorded on standardized forms to ensure that all important medical details are provided to medical care providers, whether on board or shore-based, or to officials, such as coroners and the police

This information may also be ofinterestto others, including insurers, legal representatives, or ship owners However interested these parties may be, they do not have a right to any medical information about the patient These forms, therefore, should not be used to communicate with anyone not concerned with the medical care of the crew member

Four sample forms are proposed here:

1 A ship master's report form, to be completed by the

ship's master assisted by the ship's doctor or crew member assigned to basic medical duties: this form, which should be filed in the ship's medical log, provides a brief but sufficiently comprehensive record of every medical case managed on board
