Trang 11Procedures to 0btain Radio Pratique
Application for Radio Pratique:
A When the master requires radio pratique, he should cable the Head of the Quarantine Station viaagents in accordance with the quarantine rule which stipulates that it must be done within 36 hours prior
to the respected arrival poft except Saturday, Sunday and Holiday
B The items to be reported by cable are enumerated in clause " f " hereunder
C When agents i'eceive the telegram from the master, the agents submit the Radio Pratique ApplicationForm together with a copy of the cable sent by the master
D After checking and inspecting all telegrams and papers submitted by the agents, the quarantine ities will grant the Radio Pratique if everything is in order and issue the free pratique certificate and term
author-of validity
E The master who has received the cable advising issuance of the free pratique from the authoritiesshould submit the necessary documents, crew list and passenger list to the authorities on his arrival
F ltems to be reported by cable to the authorities are as follows:
( 1) Notification that this cable is the information provided by Paragr aph-2, Article-17 of the Law cerned This identification should be noted on the top of the body as abbreviation of "RPM".( 2) Name and nationality of the vessel (the item is unnecessary in this cable)

( 4) Name of port of call and sailing date during 21 days before arrival
( 5) Name of porl and E.T.A quarantine area (local time) at destination of the vessel
( 6) Deratisation Certificate - name of the authorities issued, date of issue and kind of deratisation.( 7) Number of crew and passengers
( 8) Whether or not the vessel had transferred any passengers and/or goods from another vessel oraircraft on her voyage during 21 days before arrival In case this is affirmative, details of whichshould be described
( 9) Whether there are any patients on board or not taken ill during 21 days before the arrival In casethis is affirmative, detailed conditions of illness must be given
1 Fever accompanied by prostration or persisting for several days, or accompanied with glandular swelling
2 Consciousness disorder
3 Cough or respiratory distress
4 Any acute skin haemorrhagic rash other haemorrhagic symptoms with or without fever
5 Severe diarrhea with symptoms of collapse
6 Jaundice accompanied by fever
(10) Whether there has been any death of persons (with the exception of deaths caused by accident)during 21 days before arrival, and if this is affirmative, details of it, i.e name and age of the per-son and/or present condition of the person
(1 1) Whether rats were caught and whether there was any danger of the outbreak of black plague,and whether or not rats died abnormally during 6 days before arrival
(12) Whether there is or not goods on board tainted by any virus of quarantine epidemics
(13) Whether or not there is a doctor on board
(14) Name of the master of the vessel
