III SAFETY MANAGEMENT ORGANIZATION

2.1 Project Manager :
- In charge of the overall accountability for construction site health, safety andenvironment (HSE)
- Be accountable for achieving HSE goal
- Be responsible for ensuring subcontractors implement, administer, plan, train, andpromote the HSE plan
- Set up site HSE Plan and be responsible for their implementation beforecommencing site activity
- Use only competent personnel to work on site
- Issue, where necessary, in accordance with the policy on high-risk activities writteninstructions setting out method of and safe work
- Plan and maintain housekeeping to high standards
- Inform management of contractor, including owner’s representative immediately toenter count any accidents, incidents with the potential of injury and illnessconsequences
- Ensure all activities to site with the identified HSE problem
- Immediately remove from site any people (staff, subcontractors, or third parties)who are not willing to comply with the HSE requirements
- Provide equality of HSE training opportunity to all employees

2.3 Safety Officer:
- Assist to Project Manager in overall management and execution of HSE plan
- Conduct regular site inspections and prepare reports to Project Manager forcorrective actions
- Check and advice on work plan or procedures for HSE prior to workcommencement
- Coordinate with owner HSE section regarding to HSE activities
- Liaise with subcontractors HSE Supervisor regarding to HSE to all workersundertaken by subcontractors
- Act at the capacity of secretary to HSE committee
- Provide advice, guidance for accident prevention
- Coordinate with government authorities, especially Ministry of Labor on matters ofpertaining to legal HSE requirement
- Establish and maintain proper HSE administrative system
- Investigate all incidents and accidents and submit reports completely and presentremedial actions to HSE team of head office through Project Manager for correctiveaction

- During each event the deputy safety officer will assist in the safe and effectivemanagement of each event
- After each event the deputy safety officer will advise the safety officer of anylessons learnt which should be taken into account of in planning the next fixture
- The deputy safety officer will have a specific responsibility for monitoring theimplementation of the Club Health and Safety Policy for employees during an eventand bring to the attention of the safety officer any health and safety issues whichoccur
- The deputy safety officer shall keep the safety officer informed of how the ClubSpectator Safety Policy is being implemented and make recommendations foramendments to the policy
- The deputy safety officer shall monitor the club grievance procedure policy andbring to the immediate attention of the safety officer any grievance or disciplinaryissue affecting the event day staff

IV APPOINTMENT, AUTHORITY AND RESPONSIBILITY OF SAFETY
STAFF
- Safety Officer has been authorized with adequate power to take appropriate anddirect measures to ensure the safety of the Site in case of emergency, prevent unsafepractices and unfavourable effect on the environment and correct the inappropriateand insufficient traffic control measures or behaviors against the safety plan andrules
- Safety Officer is responsible for reviewing whether the proposed procedures andpractices in safety plans of Subcontractors are in accordance with Site Safety Planand statutory regulations
- Safety Officer shall not be removed from the Site without the written consent of theEngineer Within fourteen (14) days of any such removal or notice of intent of
Trang 8- According to the requirements of the Contract provisions and TechnicalSpecification, the Contractor will set strict criteria for the employment of SafetyOfficer and Safety Staff to satisfy the requirement of Project safety work First,Safety Officer must have a good knowledge on the laws and regulations etc of theVietnamese government on the requirement of health and safety, or comprehend andhave a good knowledge on the above laws and regulations by training; Second,every safety stuff shall be educated and trained on health and safety beforeundertaking any work to know the source of danger on site and the preventionmeasures; Finally, carry out work strictly in accordance with the approved Healthand Safety Plan, warning and reporting problem if any
V HEATH& SAFETY CONTROL WORKFLOW AND COMMUNICATION

Trang 9Safety staff are authorized to implement solution if engineer is not available, includingdirect instruction to site employees to make safe the site or take urgent direct action forsafety.
- Communication Procedures
Interaction and communication between the construction personnel on site and safetyassurance staff: According to the requirement of project, there is a billboard for everywork item on which are telephone numbers of the related safety staff who can beconnected in 24 hours of a day so that the construction personnel on site can contact withthe safety staff at any time
The telephone numbers of the Project Manager, the Safety Officer and Deputy SafetyOfficer who can be connected in 24 hours of a day shall be on the billboard to ensure thetimely communication of safety matters
A schedule of regular communications between the safety officer, safety staff, and siteengineer shall be established Each site engineer will be readily accessible by the safetystaff, and the safety staff shall be readily accessible by site engineer and workers
Interphones shall be equipped for the vertical communication at height for implementingthe main pylon and main beam of the main bridge to ensure the effective communicationbetween construction personnel on site and safety staff
- Reporting Procedures
For the health and safety problem arising from site construction, Sub -contractors andtheir staff shall report timely to the site safety staff and Site Engineer if any If possible,the site safety staff and Site Engineer shall reach a solution, and the Site Engineers shall
be responsible for implementing it immediately The site safety staff and Site Engineershall respectively report to the Safety Officer and Site Manager provided that the sitesafety staff and Site Engineer can‟t solve the problem after communication If possible,the Safety Officer and Site Engineer shall arrive at a solution, and the Site Manager shallimplement it immediately However, the Safety Officer is authorized independently toissue direct instructions to site employees and to take urgent action make safe the siteand assure safety Furthermore, if the Safety Officer and Site Manager can‟t solve theproblem either after communication, they can respectively report to the Project Manager.After the Project Manager has had the solution, it shall be noticed and execute in reverseprocedures
The Contractor shall attach great importance to the safety construction of contractors and low-tier subcontractors, knowing the means of communication betweeneach Sub-contractor and ensuring all of them carry out construction in accordance withthe regulations of the Safety Plan and all the other related laws
Trang 10Sub-VI PERSONAL PROTECTION EQUIPMENT (PPE)
1 Introduction
- All international and domestic HSE law requires the use of PPE to reduce person’sexposure to hazards when engineering and/or administrative controls are not feasible
or effective in reducing these exposures to acceptable levels
- If PPE is to be used to reduce the exposure of persons to hazards, a PPE programshould be initialized and maintained The program should contain identification andevaluation of the hazards to determine the effectiveness of the PPE
- Other factors include Supervision-Storage”
“Selection-Issue-Training-Inspection-Maintenance The use of PPE must be NOT be considered as the “last resort”, it is THE FIRSTSTEP in protecting people
- Based on hazard evaluations (conducted by supervisors) employers shall select, andhave each affected employee use PPE that will protect the employee from hazards.Employers shall communicate PPE decisions to each affected employee and selectPPE that properly fits each affected employee
- Employees shall use all PPE that may be required to maintain their exposure withinacceptable limits
- The employer will make all reasonable efforts to accommodate employees withreligious beliefs that may conflict with the PPE requirements contained within thisplan However when reasonable efforts to accommodate the employees religiousbeliefs do not provide the necessary safe working environment (without PPE), thenthe employer shall require the employees to use the appropriate PPE or the employeewill not be allowed to work in the area where he/she will be exposed to the hazardrequiring protection
2 Personal Protection Introduction Responsibility
The Contractor will take responsibility to assure that:
- The Contractor/Subcontractor shall provide the standard PPE (e.g Safety Helmet,Safety Glasses, Safety Footwear, safety harnesses, fire extinguisher, stakerope,vibro-shock, first-aid equipment, rescue equipment as well as the additionalrequirements highlighted in the risk assessment) for their visitor and employees
- The Contractor/Subcontractor shall provide the construction personnel on site whowork at height and do installation work with safety equipment, scaffold, hand-rail,work platform, crane, staircase, and other traffic equipment, hoisting equipment,lighting, signal device and protection equipment Furthermore, all such equipmentshall be tested, inspected, and maintained regularly by authorized staff to ensurethem maintain a good working condition
- When carrying out work in water, near the water and on the water surface, theContractor shall submit special safety protection measures before carrying out eachwork item, including but not limited to rescue boats, safety net, warning signs,warning lights, navigation lights, as well as search and rescue procedure, rescue
Trang 11equipment, the supervision of the workers working in, near or on the water anddetailed information on any other appropriate equipment or procedure
- Security / Safety staff/ Site engineer will stop employees who don’t wear PPE onsite
- Subcontractor shall train and require their employees to wear appropriate personalprotection equipment that is maintain in good condition Subcontractor’s employeesshall not commence work until proper protective equipment is worn at all timeswhile on site
- PPE shall be used according to respective job assignments e.g Electrical Technician– shall wear insulated gloves and helmet, and a person working at the elevated place
of steel structure shall wear belt and safety shoes with flexible non-skid soles
- All Contractor’s staff and visitors shall wear the appropriate specific PPE at all timeswhen they are in the work site
- When not in use, PPE shall be arranged and stored at designated area

VIII FIRST AID
1 First Aid Base
The first aid base will be located at the principal works area and shall consist of atreatment room fitted with a hand wash basin two treatment couches and sterilizingequipment and lockable cupboards to contain sufficient medical supplies for theContractor’s workforce, the Engineer’s site supervisory staff and any visitors to theSite In addition, two stretchers shall be stored, available for instant use The first aidbase shall contain a recovery room that shall be furnished with two chairs and twofootrests The First Aid post shall be air-conditioned, with cooling capabilitysufficient to maintain the temperature of the inside of the building at 20 degrees C

The contractor shall have adequately trained personnel available for the on sitetreatment of casualties and sick person in first -aid unit and shall provide the meanswhereby injured or sick person in first -aid unit and shall provide the means wherebyinjured or sick personnel may be removed expeditiously to hospital or receive thenecessary medical attention
5 First Aid Procedures
In case of ill health or accident happens on construction site, such as cardiac arrest,electric shock, head injury, eye injury, burns, broken limbs, drowning, snake bite etc,treatment shall be offered in accordance with the following procedures:
- The personnel on site shall notify the chief of First aid room at the earliest time Andthe chief of First aid room will lead the personnel on site who have been trained toestimate and analyze the situation found on site and conduct primary treatment andfirst aid, such as bandaging, hemostatic, artificial respiration etc The situation which
is not serious can be treated
- If in the opinion of the first-aid personnel on site, the situation is so serious that theycan’t deal with or the situation is still serious after primary treatment, the emergencycall shall be dialed and ambulance shall be connected at the earliest time (Please seethe table below: Contact Information for the Ambulance and Near Hospitals) Then,the personnel who is ill health or has an accident shall be sent to the nearest hospitalfor further first aid or treatment The personnel shall also be transferred to the moreprofessional and comprehensive hospital, such as Son La province Hospital, MocChau General Hospital for treatment and therapy if necessary And then ProjectManager and Safety Officer shall be called for the arrangement of relevant issues ontreatment and work on site
IX EMERGENCYPROCEDURE
1 General
Emergency procedure will be prepared and displayed in a prominent position in thecontractor’s site office

- The method to evacuate from the incident area will be given
- The person who is in-charge of such emergency
- Location of mustering areas and directions
- Instructions for people to walk instead of running during evacuation
- Instruction to turn-off any electrical tools and internal combustion engines and thevalves closed on any oxy/acetylene equipment if safe to do so
- Instruction to leave powered ventilation system operating, if any
- Instruction that no person can permit to re-enter unless authorized by appointedsenior project management
A decision to activate emergency evacuation must be done by In-charge SiteEngineer
Any activation of the emergency evacuation procedure must be reported to ProjectManagement and Safety Manager
2 Phuong Thanh Tranconsin Emergenc y Call Out Procedure.
2.1 Introduction
The procedures intended to be used for Phuong Thanh Tranconsin activities and must
be observed by all employees in this Project
2.2 Priorities
The Site Engineer and Safety Staff in-charge / Construction Manager and SafetyOfficer/ shall take full control of the situation Phuong Thanh Tranconsin‘s dutypersonnel who have first aid training and can provide life saving assistance includingCPR (Cardio Pulmonary Resuscitation) must perform the first aid treatment, ifrequired Immediately, after being released, the Emergency Call Out Procedure shall

- Any severe blockage of public traffic or passageway
- Anyimportant report incidents/accident to be reported
- Any significant utility damage or impact
- The emergency situations in this Project shall include:

a) Initiation
Site with staff working on day and night works, shall initiate the Emergency Call OutProcedure The Site Engineer and Safety Officer (2nd Stage) will be called, and ifthey cannot be contacted, their level will be skipped and the 3rd Stage will be alerted
In addition, the initiator will take prompt action to mitigate the situation When he isreleased he will report on:
- The situation in general in compliance with the trigger definition
- The Number of people killed / injured / involved
- Action taken by responsible site supervisory personnel
- Other vital information in brief
The Site Engineer will also report the matter to the Construction Manager and contactthe duty telephone on the situation
b) Second Stage (2nd Stage)
Safety Officer, upon receiving information will check the completeness and assess thesituation If necessary, he will travel to the Site to check himself Upon verification ofall information involved, he will initiate the Third Stage (3rd Stage) with giventelephone number(s)
c) Third Stage (3rd Stage)
The persons at the 3rd Stage (Project Manager) shall assess the situation i.e severity
of accident / incident and decide whether they will proceed with Emergency Call Out

In general rescue operations will be carried out after accidents and natural calamitieswhen injured workers are required to be rescued and evacuated from workplaces The Contractor will have provision of protective and rescue equipment as necessarybut not limited to the following:
- Rescue tripod and cable
- Safety rescue harness worn by persons working in confined spaces where this can beattached to the rescue winch cable from the tripod
- Shock absorbing lines and lanyards
- Special foldable rescue stretchers
- Face Masks (Half or Full Face)
- Self-contained breathing apparatus (SCBA)
- Fire Blankets
4 Additional Safety Equipment
There will be one oxygen resuscitation unit and compressed air escape respirator setprovided in the The Contractor site office

X ACCIDENT/ INCIDENT INVESTIGATION & REPORT
1 General
- All accidents, incidents and near misses, no matter how small they may be, shall beinvestigated according to Contractor’s incident reporting procedures The objective
is to prevent recurrence of a similar nature
- The consequences requiring in deep investigations are:
Fatal injury
Lost time injury/illness
Reporting injury
Near miss with high potential of damage or recurrence
- The supervisor shall attend all the investigation of accident that occurs at site underhis charge
- The HSE Officer, Safety Staff and Site Engineer’s in-charge, after being notified ofthe accident, shall immediately carry out on investigation but not before thecircumstances leading to the accident are under control and the victim has alreadyreceived medical attention
