1.1 Project Organisation System

1.2 Responsibility

1.2.1 Project Manager

The project Manager is responsible for the establishment of the Project Health andSafety Plan programme This Programme shall be according to the Bidder target withthe recommendations and assistance of the Deputy Project Manager No2, ContructionManagers and the safety Officer Accident prevention procedures will be included inall activities by the application of sound safety planning Specifically, the ProjectManager shall set the pace for the project health and safety programme with thesafety Officer and shall act as follows:

1 Direct the activities of the safety Team as the Project Manager

2 Promote full support of the health and safety programme

3 Delegate responsibilities for the various safety functions and authoritieswith health and safety promotion at all levels of the Project

4 Maintain active involvement in the Project’s safety status and safety morale

5 Ensure prompt investigation action on any accident investigations withproper reports completed and follow – up action taken

1.2.2 Project QEHS Manager

The project QEHS Manager (in this Project he is Deputy Project Manager No2) isappointed by the Project Manager, to be in-charge of the quality, environmental, healthand safety aspects of the activities on site with the system implemented in this Project

He will oversee the activities concerning these aspects by reviewing/discussion withthe Safety Officer or even together with the Project Manager to ensure the compliance

of the laws and regulations The specific health and safety responsibilities of theProject QEHS Manager include, but are not limited to, the following:

1 Responsibility for all safety activities on the Project and maintain liaisonwith the Construction Manager/Safety Officer

2 Establish and direct the site Health and Safety Plan programme

3 Ensure and guide Safety Officer to promote activities onsite to achieve theProject’s objectives and targets

4 Ensure the Safety Team is formed and actively involved in the health and

5 Ensure compliance to the health and safety programmes by all departments

10 Suggest/Support the Safety Officer on health and safety programmes such

as training programme for approval from the Project Manager

11 Monitor/Check any unsafe acts during site patrols

1.2.3 Construction Manager

The Construction Manager is the key figure when it concerns the monitoring of thecontractual obligations of the various contractors on the job One of these obligationswithin our total program is safety

1 The Construction Manager is responsible for the training and directing of siteengineers and office engineers which includes their safety responsibilities

2 The Construction Manager shall be prepared to accept the responsibilities foradministering the site safety programme during the absence of the Safety Officer fromthe Project Manager

3 The Construction Manager shall take and active part to the Safety Team He shallreview the specifications and prints for safety compliance and relate his findings to thisteam

4 The Construction Manager will co-ordinate activities with the Safety Officer regardingconstruction teams and subcontractor performance and take whatever steps arenecessary for proper safety compliance

5 He shall assist the safety Officer on problems or a technical nature

As administrator of the health and safety programme, he shall function as the SafetyCo-coordinator advisor and consultant to the Project Manager and supervisorypersonnel in establishing and maintaining compliance with the health and safetyprogramme Safety Officer shall be a suitable qualified and experienced person withcomplete involvement with the safety activity throughout the period of this Project

The Safety Officer’s specific responsibilities include the followings:

1 Coordinating the site safety efforts with the Project Manager and the Employer’shealth and safety requirements

2 Keeping the site management and the Project QEHS Manager informed of the progress

of the health and safety programmes and making recommendations for site relatedsafety improvements

3 Administer site health and safety policies and procedures within the framework of theBidder Health and Safety Plan particularly emphasis on potential hazardous operations

4 Proposing programmes to implement the accomplishment of short and long range ofsite health and safety objectives and to motivate all employee participations to obtainthese objectivies

5 Preparing , submitting and distributing the required safety and injury reports withrecords kept in his file

6 Maintaining a continuing inspection programme and reporting the findings andrecommendations to the Project Manager/Managers/Constractors for promptcorrections

7 Conducting and/or participating in job safety meetings

8 Supervising safety trainings in accordance with the site requirements

9 Coordinating the first aid and rescue services on site with certification to ensure theappointed employees are equipped with proper first aid know-how in the absence of anurse

10 Establishing a fire prevention programme with training on the actions andparticipation of the employees for designated duties such as informing the FireBrigade

11 Investigating all accidents, injuries, fires, property damage and other safety relatedincidents and prepare/submit the required report

12 Identify the classification of injuries if they are occupational or time losing,disabling injuries and take the follow-up actions on the control of injury cases withappropriate accident forms to be filled

13 Assisting Sub-Contractors with their health and safety program to meet the varioussite health and safety requirements

14 Notify the Project Manager and Project QEHS Manager of all health and safetyinspections with any action to be taken

15 Evaluating the needs and then, request of safety, first aid and fire protectionequipment needed on the job Ensure that safety, equipment is maintained in goodcondition at all times.

16 Check/Provide safety approval for new tools and equipment, including tools neededfor specific job

17 Effective display and maintain publicity of news of health and safety on the sitebulletin boards, posters, safety signs, banners and distribution of safety literature toinstill a good and positive attitude on health and safety environment among the people

of all levels on site at all times

18 Administer and/or monitor the site hazardous waste program

19 Administer and/or monitor the Occupational Health and Safety Act and thehazardous materials information system regulation on worksite

20 Attend pre-job meetings with Sub-Contractors and ensure the Sub-Contractorscomply to the Bidder safety policies and procedures during their work in the Project

21 Plan the site health and site safety program timely to achieve the required site healthand safety policies and procedures accordingly

22 Maintain an updated knowledge of al health and safety policies and procedures withknowledge of state provincial and local safety laws and regulations

23 Maintain updated knowledge of state-of-the-art concepts of accident prevention andcontinually contributing to upgrading the site safety effort

24 Execute effectively on any other assignments delegated by the Project Manager.Construction Manager The Site Engineers will be directly responsible for the success

of the health and safety programmes as they are in direct contact with the Contractors Their abilities to instill good health and safety attitudes to the Sub-Contractors will reflect the success of the health and safety programmes on site

Sub-Site Engineers are responsible for a wide range of activities They must plan theirhealth and safety activities with care and good effort as they have to carry out otherportions of their work programme such as cost, manpower allocation and scheduling ofthe respective work within this Project

The specific health and safety responsibilities of Site Engineers include, but are notlimited to, the following:

1 Review all work and all new personnel for safety needs Completefamiliarization with health and safety requirements is an integral part of the total jobresponsibility

2 Continual monitoring of the safety attitudes of Sub-Contractors and theirresponsibilities towards the health and safety of their trade and participations in thejob safety effort.

3 Enforce on compliance with all established health and safety regulationsbefore and during performance of the work

4 Conduct health and safety inspections and make corrections on a dailybasis

5 Ensure that proper trade health and safety training is provided

6 Initiate plans prior to the start of work to supply the tools, materials, andequipment necessary to do the job safety

7 Assist with accident investigations to ensure proper reporting anddocumentation with prompt corrections made after accident and to prevent anyrecurrences

8 Co-ordinate planning and work activities with other supervisory personnelperforming work within, or adjacent to, their assigned work areas

9 Site Engineers who have responsibility for contractor’s operations willmonitor Sub-Contractor’s compliance with the site health and safety programme.This activity will be coordinated with the safety Officer

1.2.4 Health and safety checker(Sub-Contractor’s Staff)

The health and safety checkers are key people on construction site to prevent accidentfrom happening They are the personnel at the front line, directing work activitieswhere by they are in control over the workers for the respective works which have thelikelihood of victims from potential accident Therefore, the Safety Checkers have toknow and be responsible for the followings:

1 Check their workers at the start of work daily to make sure these workersare ready to carry out their assigned tasks from their knowledge /information oftheir workers under their charge and also the physical and mental state of eachworker daily

2 Observe and monitor continuously all workers on their knowledge andcapabilities on their works and health & safety issues

3 Follow-up continuously with site inspections to ensure health and safetyinstructions are carried out such as personal protective equipment required to beused and in proper order

4 Give specific health and safety instructions with each job assignmentaccording to each worker’s knowledge of job with related impacts to theenvironment, health and safety.

5 Ensure workers under their charge use the proper well-maintained tools andequipment for each job

6 Check for any incident at the end of the day daily and report to safetyOfficer whenever necessary

7 Inform Safety Officer if unsafe acts or conditions found which they are notresponsible

1.2.5 Operator / Workers

Operators or Workers working with tools, plant, equipment, etc are responsible fortheir health and safety with others working around them Therefore, there should besufficient instructions provided to them They shall be responsible for the followings:

1 Use proper, correct and well-maintained tools and equipment for the jobs

2 Use protective measures such as safety helmets, safety boots, goggles, safetybelts, etc must be worn whenever required

3 Maintain and keep tools in good condition at all times

4 Report defects in plant or equipment or any potential risk to health and safety toSupervisor

5 Avoid improvising work which entails unnecessary risk

6 Refrain abuse of welfare facilities

7 Suggest ways for eliminating hazards

2.1 General

1 No employee shall be required to work in surrounding or under workingconditions which are unsanitary, hazardous or dangerous to his / her life, health andsafety

2 The Bidder shall be responsible for initiating and maintaining a health and safetyprogram which will comply with the Contract, Engineer’s Representatives andStatutory requirements

3 The Bidder Safety Program shall provide frequent and regular safety inspections

of the worksites, material and equipments by competent person designated by theBidder Results of findings shall be submitted to the Engineer’s Representative

2.2 New Employees Indoctrination

Each employee shall be provided initial indoctrination and such continued training willenable him to perform his work in a safe manner

Indoctrination and training shall be based on the health and safety program of theBidder or government agency and shall include but not limited to:

1 General health and safety policy and pertinent provisions of the Bidder ProjectHealth and safety control plan

2 Requirements for employees and project safety

3 Employee’s responsibilities for property and the safety of others

4 Employee’s responsibilities for reporting all accidents

5 Medical facilities and required treatment

6 Procedures for reporting or correcting unsafe conditions or practices

7 Safe clearance procedures

8 Fire and other emergency procedures

2.3 New Employees Indoctrination

1 Safety meetings scheduled on regular basis shall be held at least once a month forall Site Engineers on the Project to review past activities, to plan ahead for new orchanged operations and establish safe working procedures for anticipated hazards

2 Everyone required to enter confined or enclosed spaces or atmospheres withimmediate danger to life, shall be instructed as to the hazards involved andprecautionary measures to be taken Training by qualified person on the use andcare of such emergency and protective equipment shall be conducted All the Bidderstaff shall comply to all specific regulations in dangerous or potentially dangerousareas

3 All employees who have to handle poisons, caustic or other harmful substances,are required to receive instruction regarding the safe handling and use Informationand knowledge on the potential hazards, personal hygiene, protective equipment andother personal protective measures involved or required

4 Personnel who may be required to use the rescue of life saving equipment must

be familiar with the locations of the equipment and knowledge with training onusing them

5 Personnel required to provide or receive signals, must be familiar with the properuse of the signal system

6 Personnel required to handle or use flammable liquids, gases or toxic materialsmust be trained in the handling, storage and use of them with specific protectionrequirements

7 Whenever the employees are likely to extreme temperature or exceedatmospheric pressure, instructions on recognition, potential hazards, avoidance ofinjury and first aid procedures to be applied for any incident

8 The Bidder shall provide a trained fire fighting team on site if required byAuthority, to ensure the protection of life and property

9 The Bidder shall provide training on the handling of emergency situations thatmay arise from the use of equipment in the Project

10 Instruction to use and operate any heavy equipment will be given by qualifiedrepresentative of the Bidder and personnel required to use these equipment shalldemonstrate their proficiency in the operation process

11 Any personnel required working with energized power and transmission linesshall be instructed and trained in the live line bare-hand technique and safetyrequirements before allowed to use

3 Health and Safety Meeting

3.1 HS Committee meeting

The Bidder Health and Safety Meeting will be held on monthly basis A team shall

be formed which is known as Health and Safety Committee The attendees for thismeeting shall be the project Manager, Deputy Project Managers, ConstructionManagers, Safety Officer and relevant staff with the agenda of this meeting but notlimited to them, as follows:

1 Health and safety target/Program review

2 Summary of monthly health and safety report

3 Monthly Safety inspection

4 Any other matters related to Health and Safety

3.2 Tool box Meeting

Tool Box Meeting will be conducted by Safety Officer and safety Checkers on aregular basis to address the safety related matters with the workers, issues will beraised or identified during the safety inspections and reminded at the Tool BoxMeetings, Safety Officer will prepare the items for these Tool Box, Meetings SiteEngineers will take the necessary action for those issues mentioned at these ToolBox Meetings to ensure conformity of such issues are carried out.

4 Safety Reports and Notification of Accidents

4.2 Notification of accidents

1 Any injury or accident should be investigated and reported to theEmployer’s and the Engineer’s Representative if it involves any of thefollowings:

a Injury or accident that resulted in >3 days unable to work

3 Further to informing them by telephone, the Bidder have to provide byfacsimile the report in advance on the relevant issue within 3 hours to theEmployer’s and the Engineer’s Representative The complete detailed reportmust be submitted by the Bidder to the Employer’s and the Engineer’sRepresentative within 24 hours

5 Personal protective apparel and other site safety protection

5.1 Personal protective apparel

Personal protective gears like protective footwear such as safety boots, rubber

protection All points of entry for hard hat must be identified with the sign Visitors

to site must also adhere to the safety requirements at the site

1 Protection to eyes and face for work that may cause injury due to physical,chemical or radiation agents

2 Reflective or high visibility material should be worn for staff exposed tovehicular traffic e.g Signalmen, spotters, inspectors servicemen, etc

3 Protective nets for overhead protection shall be provided wherever thepublic or workers are subjected to injury due to falling objects

4 Work near protruded vertical reinforcing steel shall not be permitted unlessprovision to eliminate possible hazards is carried out

5 Protection to ears with ear plugs or ear muffs are required for soundtransmitted at working area that exceeds 85DbA

6 Insulating personal safety gears such as insulating gloves, insulating linehose, etc to be provided for electrical staff

5.2 Specification

Specification for the common personal protective gears is as shown below:

1 Any headgear: <240g non-metallic material withstand impact of <385kg

4 Hand Protection: Leather gloves to cover up to wrist and fingers Material

of mixed metal foil

5 Footwear Protection: Safety shoes with steel cap at tip of shoes withstandpressure of>445kg

5.3 Other Safety Equipment.
