START treadmill: ETT if two CAD risk factors

STOMACH: Diabetic gastroparesis: Reglan KIDNEY: Urine albumin / creatinine ratio < 20,

check annually, treat with ACE and/or ARB

GENITALS: Ask about sexual dysfunction ASSpirin

(ASS)Immunizations: Pneumovax-23, Flu, Td FEET: Examine every visit,

monofilament exam q 6 – 12 months, podiatry referral if deficits

HUG A LOG: HgA1c q 3 – 4 months,

evaluate glucose log q visit
