INSTRUCTIONS FOR COMPLETING THE INTERNATIONAL STUDENT FINANCIAL STATEMENT
Read the following instructions carefully before completing and submitting this form
All international applicants must document their ability to meet all educational and living expenses for the entire period of
their intended study before this university can issue a Certificate of Visa Eligibility (form I-20) Please fill out the form, click
"File" > "Save As," select a location on your computer and save it
Section I: Answer all questions completely
Section II: Applicants are responsible for all costs of attending the university (4-5 years for freshmen; 2-3 years for
transfer students) College costs are subject to change and increase an average of 5 percent annually Check the
appropriate statements indicating where your first year of funding will come from (multiple sources are acceptable) You
must include supplementary documents/proof as indicated, and attach originals, not photo copies, in United States
Dollars (USD) or shown at the applicable conversion rate All documentation must be current and dated within 3 months
of the date of your application to SUNY Oneonta
Sources of Funding:
Personal: Signature on this form and a bank statement of account in US dollars
Family/relative/sponsor: Completed Affidavit of Support and a bank statement of account in US dollars
Scholarship: Official scholarship award letter from institution awarding the scholarship
Award letter must include name of applicant, amount of money for each year
of study, beginning and end dates for the award and any academic requirements for the award (degree or major), if applicable
Government/Company: Official letter stating the name of applicant, amount of money for each year of
study, beginning and end dates for the award and any academic requirements for the award (degree or major), if applicable
Loans: Official letter from credit institution indicating amount of loan and approval
SUNY Oneonta reserves the right to require additional financial documentation

WHAT DOES THIS AFFIDAVIT MEAN?
By completing this affidavit, you are swearing to the U.S government that you will provide this student with a specific amount of money, from your own financial resources, for every year that he or she is going to study at SUNY Oneonta and live in the United States You are also proving that you can afford the support you are promising with the
documentary evidence you have attached
HOW TO COMPLETE THIS FORM:
• Fill this form out in English (or have it officially translated)

• Attach the supporting evidence listed below The affidavit will not be accepted without the required supporting evidence
• Give the student the original documents and have him or her submit them electronically or make photocopies to mail or fax to us with the application for a Form I-20 The student will need to keep the originals to present to the consul at the visa interview
