Dear Parents,

Welcome to the Germantown swim Team In addition to the monthly swim meets, there are several activities we try to do together as a team We have a kick off party at the beginning of the season in September, Halloween Activities,

A Christmas Party, and our awards banquet in the Spring All of these activities are announced through team emails

Included in this packet you will find an explanation of fees, a fee chart, detailed instructions on how to register online, practice schedule, an explanation of USA swimming coverage, and other ways to connect online

Explanation of Fees—

Joining the swim team includes an annual fee and session Fees :

 The annual Registration fee of $225 covers your swimmer’s USA

registration Our swim club, coaches and swimmers are covered by USA swimming insurance In order for the club and coaches to be covered, all swimmers practicing or competing in swim meets must be registered with USA swimming USA swimming also provides additional coverage for its swimmers (see attached) Of the $225, $66 goes to USA swimming, and

$45 goes to the City of Germantown for annual dues The remainder of the fee covers the team t-shirt, swim cap, and team parties (This fee is

reduced for the period of March 1-August 31 to $115)

 The second set of fees is based on the swim group your child is assigned

to, and paid directly to the Germantown Athletic Center (the fee schedule

is attached) These fees are paid at the beginning of each session (fall September 1, winter December 1, Spring—March 1, and Summer—June 1) These fees cover coaches’ salary and pool maintenance

 Completed Online Registration (instructions provided)

 Signed Team Release and Waivers Form (included in this packet)

 TN Concussion Form (found under documents tab on team website-signature and initial page must be printed)

 Germantown Athletic Club session fee (2016-2017 fee sheet attached)—visit the GAC front desk to make this fee payment

Please turn in all forms and checks to Coach Scott, or they can be placed in the blue box located on the pool deck by the bulletin boards, or mailed to:

Registration Instructions For New GST Families

1 Go to gstswimming.com, click on the START REGISTRATION tab, which can be found on the left side of the web page

2 Review the Registration Requirements and print the necessary forms Click Continue

3 Click Create New Account, enter your email address and birthdate Click Continue.

4 2016-2017 GST registration page will open Fill in all fields

Please Note: Fill in the field marked “Alternate Email 1” with your primary email address , to receive team communications Click Continue

5 Click the Add Member tab on the top right

Enter your swimmer’s information

For Location: Select Germantown Pool

For Register to This Group: Select the appropriate Group (Bronze, Silver I, Silver II, Gold, Senior) Click Add

If you have more than one swimmer, repeat the process Then proceed to the next step

6 Read all electronic documents and, if you agree, check the boxes (Digital photography and Email are optional) Click Continue

7 Make sure all information is correct Select method of payment as Credit Card or Check (To change the method of payment click on drop down box) Click Proceed to Checkout

8 The total charges will appear at the top of the page If you agree click Submit Registration

If paying by check make payable to Germantown Swim Team Checks and other required

registration forms can be placed in the blue box on the pool deck by the bulletin boards, or they can

be mailed to:

9 Registration is now complete You will receive a Registration Receipt by email Once payment has been received you will receive an email to set up your personal account on our website, and an email to verify your email address Go to your account online and check your email accounts and phone numbers to confirm they are verified

10 Please remember, there are two payments for separate fees, $225.00 which is the team

registration fee and paid directly to Germantown Swim team and swim session fees that are paid at the front desk to the athletic center

Membership Protection

USA Swimming’s policy for membership protection is:

 To provide safety education for its membership;

 To provide excess accident medical protection for USA Swimming members who may suffer injuries

while participating in insured activities;

 To provide evidence of financial responsibility so that USA Swimming clubs can conduct insured

activities.

To implement this policy, USA Swimming has adopted two major programs:

 Excess Accident Medical Protection

 General Liability Insurance

These two programs are intended to provide reasonable protection for USA Swimming athletes, non-athlete members and clubs

When Coverage Starts:

When Coverage Is In Effect:USA Swimming members are covered while participating in a USA

Coverage is a benefit of membership in USA Swimming and begins upon receipt of the completed membership

application form with appropriate fee and acceptance by the LSC Registration/Membership Chair or its designee Swimming supervised, sponsored, sanctioned or approved event including:

  Competitions and meets;

  Organized practice sessions;

  Approved social and fund raising activities;

  Travel to and from competitions, meets, events, organized practice sessions, approved social

and fundraising activities at the direction of a coach or club board of directors;

  OVC events for USA Swimming Officials only

USA Swimming Insurance Coverage for the Club

Question: What must a club do to have the benefit of USA Swimming’s insurance coverage?

Answer: A club’s USA Swimming insurance coverage is in effect when all three of the following conditions are met:

1 The club is a member in good standing of USA Swimming.

2 All participants swimming for the club are athlete members of USA Swimming

(exception: see Tryouts)

3 All coaches, whether they are full-time or part-time, are coach members of USA Swimming with current certifications, education and background checks as required by USA Swimming

If all three of these conditions are met, the USA Swimming club has the benefit of USA Swimming’s liability coverage for insured activities

Germantown Swim Team Release and Waivers

This form must be returned with copy of child’s birth certificate

In accordance with the GST Mission Statement and Vision Statement, and in association and

compliance with USA Swimming, and State of Tennessee Law, GST has adopted several policies and guidelines aimed at the protection of our children, coaches and parents The intent of these policies and guidelines is to communicate and provide parameters of what is expected of all involved with GST Where possible, we have provided age appropriate versions of certain policies It is a

requirement of participation with GST that all registered GST Swimmers, their parents and coaches acknowledge all of these policies and guidelines
