PREPARATION AND REQUIREMENTS FOR A SUCCESSFUL OFFICE VISIT:
Arrive 15 minutes before your appointment time to allow for processing paperwork and registration check in
Bring your insurance card with you and picture ID
Remember to complete the new patient forms and bring them with you along with your radiology reports, films and or CD
Call your insurance company to verify our doctor participates in your health plan and ask them if you need an insurance referral to see a specialist When your health plan requires a referral you need to contact your primary care doctor at least 5 days in advance to allow sufficient time for them to process your request Remember to bring the referral with you to the visit
When your insurance plan requires referrals to specialists and you do not have one for the visit, you will be charged the consultation fee for service
