OBJECTIVES: This is a knowledge-based activity whereby the learner will be able to:

1. (P) Review the pathophysiology and classification systems used to define heart failure

2. (P) Identify current pharmacologic treatment recommendations supported by ACC/AHA Guidelines

3 (P) Identify opportunities for community pharmacy interventions, including strategies for improving transitions of care

4. (P) Discuss case examples utilizing learned concepts
