This Resource Directory is provided by the Illinois Service Resource Center

(ISRC) The purpose of the directory is to assist parents, teachers, service

providers and others with a guide to services available to children who are deaf, hard of hearing, or visually impaired and exhibit behavior behavioral or emotional challenges.

The ISRC operates under a grant from the Illinois State Board of Education,

funded by federal IDEA discretionary funds Its purpose is to fulfill the

recommendations of the Interagency Board for Children Who or Hard of Hearing and Have an Emotional or Behavior Disorder legislation mandating creation of a center of children and adolescents through age 21 who are deaf, hard of hearing,

or visually impaired and have behavior disorders.

Among its many responsibilities, the ISRC will:

 Establish and maintain a statewide identification and tracking system,

 Provide case coordination, referral and consultation services,

 Provide technical assistance for existing programs and providers,

 Provide training for existing programs and providers,

 Monitor, evaluate and assess statewide resources,

 Provide limited direct services.

General Purpose: To provide mental health services to deaf and hard of hearing individuals

and their families.

Geographic Location: Chicago and suburbs

Geographic Area Served: Chicago and suburbs

Age Range Served: All ages

Eligibility: Individual or family in need of mental health services, with at least one

deaf or hard of hearing family member.

Intake Procedure: Call to set up an intake appointment.

Fee Structure: The cost of services depends on family size and income

Most Public Aid and insurance plans pay for part of all of the cost of services

Interpreter Policy: We can communicate in English, ASL, Signed English and

Spanish.

Services Available: Individual, Family Group and Couples Therapy, Clinical and Psychiatric

Evaluations, Medication Monitoring, 24 hour TTY Crisis Line (773-296-3737), Consultation to schools and social services agencies, Community/Educational Programs, Intensive Case Management, Liaison Case Management (helping to integrate Chicago-Read patients into the community), and Psychiatric Clinical Deaf Liaison Services.

General Purpose: The unit provides inpatient psychiatric services to Individuals

who are severely emotionally disturbed and/or carry a primary forensic diagnosis

of mental illness

Geographic Location: Central Illinois

Geographic Area Served: 18 counties including: Hancock, Schuyler, Adams, Brown, Cass,

Pike, Morgan, Scott, Greene, Calhoun, Jersey, Mason, Logan, Menard, Sangamon, Christian, Macoupin, Montgomery

Age Range Served: 13 years to 17 years

Eligibility: Clear danger to self or others or inability to care for self

Intake Procedure: Screening is done by the appropriate Mental Health Center in the community

The mental health center then contacts Cathy Smith or admission.

Fee Structure: As a state operated facility, McFarland's fees are based on ability to pay - either

private, insurance or other funding - with a sliding scale

Interpreter Policy: An interpreter is available for one to one services

Services Available: Educational, Evaluation, Psychological Counseling, Medical, Speech and

Language Therapy, Vocational Training, and Recreational.

General Purpose: To assist people with disabilities to live and work successfully in the

community - through quality vocational, residential and educational options; substance

abuse prevention and treatment; employment services; health care; and advocacy

Interpreter Policy: Addiction Recovery of the Deaf has counselors and interpreters skilled

in sign language and interpreting; Anixter Center has a manager of interpreting services for other programs; some direct service staff and residential service staff can interpret, depending

on the program.

Hands and Voices

Hands & Voices is a non-profit, parent-driven organization dedicated to

supporting families of children who are deaf or hard of hearing We are

non-biased about communication methodologies and believe that families can

make the best choices for their child if they have access to good information

and support Our membership includes families who communicate manually

and/or orally From American Sign Language to cochlear implants, our

organization represents people from all different approaches to, and experiences

with, deafness or hearing loss We have local chapters comprised mainly of

parents along with professionals

Guide By Your Side (GBYS) is a program that provides:

Unbiased information

One on one support and resources

Trained Parent Guides to talk and/or meet with you.

What are Parent Guides?

Parent Guides are adults who have children with a hearing loss.

They provide unbiased information and resources regarding communication,

educational and technological choices.

They help families identify their options…not tell them which options to choose.

They support families Support is all about listening, not judging, and without bias.

They help families navigate through the many systems and services available in Illinois They work as a team with professionals offering a well rounded care system for families.

General Purpose: The Illinois School for the Deaf provides a comprehensive educational

program for Illinois deaf and hard of hearing residents between 3 and 21 years of age, in

addition to a parent/infant program for children 0 to 3 years of age A wide range of related services are available including residential, vocational/technical, athletic, recreational, and health programs.

Eligibility: Parent/guardian must be a resident of the state, student must have identified

hearing loss and the school must be able to write and appropriate educational plan.

needs at the Universal, Secondary (Targeted) and Tertiary (Intensive) level needs To provide training, technical assistance and resources for parents and educational teams of students with

a hearing loss or visual impairment and emotional/behavioral issues.

Age Range Served: 0-21 years

Eligibility: Educational program or family with a deaf, hard of hearing, or visually impaired child Intake Procedure: Contact ISRC for information

Fee Structure: There is no fee for information, referral, case coordination or helpline calls

There may be a fee for evaluations.

Interpreter Policy: There is at least one person available at all times who can sign and provide

services on a one to one basis

Services Available: Information and Referral, Resource Directory, Technical Assistance and

Eligibility: The outpatient program is open to anyone (hearing or Deaf)

The Deaf/HOH/Deafblind program is for Deaf or Hard of Hearing or Deafblind individuals.

Tuesdays: 12:30 to 4 p.m.

Fridays: 12:30 to 4 p.m.

Fee Structure: Varies, Medicaid Accepted

Interpreter Policy: All Deaf Program staff are fluent in ASL or signed English and can provide various levels of communication needs for this population The program has a full time ASL/Spanish interpreter on site.

Services Available: Individual psychotherapy, family therapy, psychiatric services (evaluation,

medication, and education).

Eligibility: There are no eligibility requirements

Intake Procedure: Call the clinic office to schedule an appointment

Fee Structure: An hourly rate of sliding scale based on income level Our goal is to provide

services that the families or individuals are in need of, so we are very flexible regarding cost for services.

Interpreter Policy: There is at least on person available at all times who can sign and

provide services on a one to one basis

Services Available: Individual, Couple and Family counseling, Speech and Language

Therapy, Parent/Infant, Sign Language Classes, Disability Awareness Seminars, Job

Coaching, Advocacy, Career Counseling, Independent Living Skills Counseling, Tutoring, support Groups, Information and Referral.

Additional Information: The Clinic provides services in three emphasis areas: Speech

General Purpose: The Center and School is operated by The Illinois State Board of

Education The primary goal of the Philip J Rock Center and School is to help people

who are deaf-blind achieve maximum independence based on their individual potential Our vision is of the community of the Philip J Rock Center and School, which consists

of the parents, educational staff, and support staff, working together so that all students learn the skills needed to become valued and accepted members in their home, local,

and future adult communities.

Geographic Location: Northern Illinois

Geographic Area Served: Illinois

Age Range Served: The residential school offers one of the most intensive,

comprehensive educational programs available for children who are deaf-blind in the 3- to 21-year-old age group.

Eligibility: Parent/guardian must be a resident of the state, student must have identified

hearing loss and the school must be able to write and appropriate educational plan.

Intake Procedure: Call to discuss.

Hours of operation: 24 hours a day.

Services Available: The Center provides a wide range of services on a statewide basis

Services are provided in schools, medical settings, early intervention centers and homes Services for Children include: Community Outreach Programs; Counseling; Daily Living Skills/Independent Living Skills Training; Education Services; Employment/Job Training; Health (audiology; general medical services; occupational, physical and speech therapy; and referrals for and contracts for other medical services); Summer/Day Camps and

After-school Programs; and Travel/Orientation and Mobility Services for Professionals include Consultation and Technical Program Assistance for children who are deaf-blind.

General Purpose: Project Reach provides technical assistance, information, and training

to address the early intervention, special education, related services, and transitional services needs of children with deaf-blindness and enhance state capacity to improve services and outcomes for children and their families Services are coordinated with other state agencies responsible for providing services to children who are deaf-blind.

to increase the identification of eligible children and those suspected of having dual sensory impairments In addition, deaf-blind specialists conduct informal functional vision and hearing evaluations They assist in the process of making referrals for formal

diagnostic evaluations Needs assessments are distributed to family members and

professionals which allow them the opportunity to identify areas of need for consultation services and various forms of technical assistance.

Thresholds Bridge Hearing Impaired Program

General Purpose: Thresholds Deaf Program’s mission is to create the opportunity for deaf

adults with serious and persistent mental illness to live a life of dignity and independence in the community

Geographic Location: Group homes and community outreach programs located in both

the north and south sides of Chicago

Geographic Area Served: Chicago (North and South)

Age Range Served: 18-70 years old

Eligibility: Deaf adults with a diagnosis of serious and persistent metal illness

Intake Procedure: Psychiatric/psychological evaluations, hospitalization records, police

reports, social history sent for evaluation

Fee Structure: Call for information

Interpreter Policy: All staff uses sign language (60% of the staff are deaf)

Services Available: The Thresholds Deaf Program included a variety of residential settings

based upon required level of supervision; assertive case management, home visiting

program; staff psychiatrist and nurse who meet with the members regularly; job readiness training, specific vocational training and job placement; evening, weekend and holiday

programming – Thresholds Deaf Program provides services every day of the year; all program activities and locations designed exclusively for the deaf (Deaf persons with special needs are also accommodated); academic education; social relationships and enduring friendships
