Vision, Long Term

Disability Insurance

The University of Arkansas is offering an open enrollment for medical, dental and vision

plans The open enrollment period begins on November 2, and ends December 1, 2015 All changes made during this period will be effective January 1, 2016

Enrollment forms are enclosed or available for download from the Department of Human Resources’ website http://ualr.edu/humanresources/current/benefits/open-enrollment-2016/ Forms may also be picked up in the Department of Human Resources

This is a once-a-year opportunity to make changes to your benefits After December 1,

2015, you can only make changes if you are within 30 days “of a qualifying life event” (marriage, divorce, birth, etc)

All forms must be returned to Human Resources by Tuesday, December 1, 2015.

LATE FORMS WILL NOT BE ACCEPTED.

TIP:

Open Enrollment is not

offered every year and

future open enrollment

periods are not

guaranteed.

UMR will mail

insurance identification

cards to all health plan

enrollees’ home

address

Through December 1, 2015, you can make the following changes:

 Enroll for the first time in medical, dental or vision coverage;

 Add or delete eligible dependents to/from medical, dental or vision plans;

 Cancel medical, dental or vision coverage;

 Change between Classic and Point of Service medical plans;

 Change vision insurance between the Basic and Enhanced Vision Plan;

Enroll in Flexible Spending Accounts (Dependent Care and/or Health Care) This is a positive election and you must re-enroll each calendar year

 Change medical, dental and vision premiums between after-tax and pre-tax

No evidence of insurability, pre-existing condition exclusions, or waiting periods apply

If you are currently enrolled in the medical, dental or vision plan and do NOT want to make any changes, NO action is required; your current coverage will remain the same This will include after-tax and pre-tax contributions.

2016.

Medical Changes (Classic/In-Network Plan)

 Deductibles will increase from $750 to $1250 (individual); and from $1500 to $2500 (family)

 Out of pocket (OOP) maximum $5250 (individual) and $10,500 (family) OOP is the maximum combined deductible, co-insurance and co-pays you will pay in a calendar year Family OOP maximum requires two individual family members to meet the individual OOP maximum See Medical plan comparison for additional information – footnotes: (c)

 Participants who participated in the Health Screening and took the Health Assessment by 09/30/2015 will receive the $1400 reduction in their 2016 Maximum OOP and $2800 reduction for family

Dental Plan

Remember that

covered individuals

covered in full each

year.

The Dental plan offers expanded preventative care for individuals with diabetes, heart

disease and periodontal disease and will now cover nitrous oxide sedation For a listing

of participating dentist, visit Delta Dental’s website; www.deltadentalar.com There is no increase in Dental premiums for 2016.

Dental FYI

 The dental plan provides for two annual exams and cleanings, covered in full, you pay $0 co-payment If you are diabetic or have heart/renal disease, additional cleanings and exams are available at the same $0 cost Using your dental preventative services you also trigger the plan carry-over which will credit you with more dental plan dollars to be used in a future year Your plan maximum could be increased as high as $3000, versus the normal $1500 annual benefits limit in a calendar year

 Dental rates are listed on the attached 2016 Benefit rate sheet Benefits are

described in the Dental Schedule of Benefits under the 2016 Open Enrollment

section on the Human Resources’ website at www.ualr.edu/humanresources

The vision plan offers a Basic or Enhanced Plan For comparison information of the two

plans, please visit the Human Resources’ website for information under vision coverage

Your benefits are greater if you use a Superior Vision participating provider Check “Locate

a Provider” on Superior Vision’s website at www.superiorvision.com There is no increase

in Vision premiums for 2016.

Changes you can make by December 1, 2015

Long Term Disability

University provides benefit

coverage for employees’

salary up to $20,000

Employees may elect

optional coverage to cover

salary in excess of $20,000

Disability benefit provides

60% of annual salary.

The University’s Long Term Disability program is administered by The Standard You can elect to cover your salary in excess of $20,000 (maximum covered salary $100,000 for Long Term Disability) During the open enrollment period, employees can enroll in optional long term disability without providing evidence of insurability Enrollment for the option long term disability program would provide an employee 60% of their total salary if a long term disability occurred Payments will be made to the employee until age

65 (benefit payments are extended for a limited period beyond age 65 if disability

occurred at age 65) During this special open enrollment there are no health questions

to answer However, if you sign up during this open enrollment, your plan will be subject

to a 12 month pre-existing condition exclusion period Pre-existing means that you had symptoms, took medications, had treatment, or were diagnosed during the six month period immediately prior to your 1/1/2016 effective date of coverage

Employees wishing to enroll in Long Term Disability Insurance must request and complete

a UA Group Change form This form can be obtained from Human Resources Rate calculation is available on the Human Resources’ website

Enjoy discounted group rates and the convenience of payroll deduction by contacting these carriers for information on individual insurance policy The coverage you elect and the cost of that coverage is determined by you and the carrier The carrier will advise Human Resources of the payroll deduction amount

Long Term Care

Insurance - CNA

Long Term Care Insurance is

not simply nursing home care

for the elderly It

encompasses a variety of

services for people who need

assistance with the ordinary

activities of daily living,

including home-based care.

Long Term Care – SPECIAL NOTE: New enrollment opportunities for the long-term care

program will end on January 31, 2016 The University will phase out payroll deductions for the long-term premiums Participants may continue their long-term care insurance through CNA through bank draft or other direct payment with the vendor, CNA Long Term Care Insurance is not simply nursing home care for the elderly It encompasses a variety of services for people who need assistance with the ordinary activities of daily living, including home-based care Contact CNA at 1-877-777-9072 or

be extended to family members including parents and grandparents

AD&D Insurance is purchased in $25,000 increments from a minimum of $25,000 to a maximum of $300,000 AD&D will pay a benefit in the event of a death or

dismemberment caused by an accident

IMPORTANT NOTE: Health Care Flexible Spending Account increases to $2550.

This is the annual enrollment period for the flexible spending accounts (Health Care and Dependent Care Reimbursement Accounts) You may want to consider enrolling in the Health Care FSA, if you have out-of-pocket medical expenses such as braces, prescriptions and insurance co-pays, insurance deductibles and co-insurance Eligible expenses include medical, dental or vision expenses not reimbursed by an insurance plan for the employee, eligible spouse (if filing a joint tax return) and eligible dependents

The minimum annual contribution is $120 and the maximum contribution is $2,550

An FSA Debit card is issued with enrollment in the Health Care Reimbursement plan (please retain your current debt card if enrolled in previous plan years).

Consider the Dependent Care FSA for qualifying daycare expenses for dependent children under the age of 13 or elderly persons with a disability as long as they are claimed as your

dependents for federal tax purposes The maximum annual contribution is $5,000 Important CHANGE regarding your FSA:

Please remember that the “grace period” was removed and the Rollover option was added to the Healthcare FSA beginning in the plan year 2015

$500 rollover for healthcare FSA – This benefit will continue for the plan year 2016 You will have until December 31, 2015 to incur expenses that will apply to your

2015 Healthcare FSA plan Unused 2015 Healthcare FSA monies (not to exceed

$500), will transfer to your 2016 plan Unused amounts in excess of $500 will be forfeited Transfer amount should not be included in your 2016 election amount The transfer benefit may ease your fear of the “use-it-or-lose-it” rule

 There is a grace period for the Dependent Care FSA account Employees have until 3/15/16 to incur expenses that will apply to their 2015 Dependent Care FSA All expenses that should apply to the 2015 Dependent Care account must be filed with UMR no later than March 31, 2016 There is no carryover for Dependent Care Accounts; 100% of unused amounts will be forfeited

To comply with Federal health care reform reporting requirements, employees will be required to provide Social Security Identification Numbers for spouse and dependents who participate in the University’s health plan The government will use the information collected to identify those individuals who have health coverage or who should be purchasing health coverage through the health care marketplace

Eligible spouse and dependent participants in the University benefit plans:

 The lawful spouse of an Eligible Employee; as defined by the laws of the State of Arkansas

 Each Child of the Eligible Employee from birth until the date on which they attain

the age of twenty-six (26) years Child includes (in additional to your natural child)

the following:

o Adopted Child for whom a petition for adoption has been filed or the final court order has been issued;

o Step Child for whom you are legally responsible;

o Children can only be covered under one employee’s plan if both employee and spouse work for the University of Arkansas

If you have a child under the age of 3, and wish to enroll them in your dental and/or

vision plans, you must enroll them 30 days before or after their 3rd birthday or during the open enrollment period

Resources by the deadline of December 1, 2015 to be effective January 1, 2016;

 Social Security numbers for covered dependents are required for medical insurance;

 Double check FSA elections for enrollment under the correct account (health or dependent care) and verify the amount elected is correct;
