The Case Manager / Host Home Recruiter has an exciting opportunity to provide services to homeless youth in Santa Cruz County by matching them up with caring adults who are willing
to take them into their homes for stabilization and support The Case Manager will market the program to families throughout the county to recruit homes, train them and match to youth referred through the Homeless Management Information System This role is responsible for retention of host homes as well facilitating trainings and certifications for host home members
In addition, will provide case management to clients assigned to the program
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Program Development: Areas to develop include:
o Recruitment, screening, ongoing support, and retention of hosts
o Communication practices between partner organizations to assure needs and concerns of youth and hosts are being addressed in a regular and systematic way
Program Implementation: Plan and execute activities to implement the Host Home
Program This includes:
o Activities to recruit, screen, support, and retain hosts
o Assessment, intake and weekly case management of youth placed in host
homes or other shared living options
o Regular communication and collaborative meetings with youth case management partners
o Obtain training in the Santa Cruz HMIS system to manage client referrals and update status of placement options
QUALIFICATIONS:
Bachelor’s degree from a four-year college or university; or equivalent combination of education and experience Experience in recruiting host homes (rooms) Knowledge of the Santa Cruz county homeless youth community, street outreach focused on youth and LGBTQ community, issues faced and cutting edge solutions and resolutions with prior experience planning and implementing outreach activities Must be punctual, efficient and organized; effective and professional under pressure Ability to balance excellent client care with meeting deadlines and ensuring positive program goals and outcomes Experience providing services
to clients of various racial, ethnic, socioeconomic backgrounds or sexual identity who are overrepresented in homeless populations as well as working in different cultural environments
An understanding of humility and respect for working with homeless youth and families Excellent communication skills - must be personable and informative when presenting BWC programs and services Valid California driver's license with access to a personal vehicle, Fingerprint/background check and TB test required
