2020 Scholarship Application
Requirements & Instructions Eligibility
1 Be a graduating senior from a high school in South Miami-Dade County.
2 Be a U.S Citizen or an eligible non-citizen.
3 Be a resident of the Homestead/South Dade area.
4 Have attended a local area high school for the past two (2) years and a member of Key Club.
5 Have an unweighted grade point average of 3.0 or better.
6 Be a Key Club member in your school.
7 Have completed community service IN EXCESS of the required minimum needed for
graduation
8 Be accepted or pending acceptance as a full-time undergraduate student in an accredited
university, college, community college, vocational/technical school
9 Pursue a recognized course of study.
10.The completed application (pages 1-4) and all additional information MUST be mailed to the
above P.O Box and be post marked no later than Friday, May 15th
Value of Scholarship
Each scholarship awarded will be in the amount of $1000.00 A maximum of five (5) scholarships awarded is based on factors including but not limited to grade point average and
community service Scholarship monies may be used for tuition, textbooks, room and board, or
other necessary school-related expenses
Required Documents:
1 A completed application Failure to submit a complete application may result in automatic
disqualification Applications received after the deadline will not be accepted.
2 Academic information, including grade point averages, test scores and other academic
achievements certified by the schools CAP advisor or school principal (use page 4 of application)
3 Two Letters of Recommendation from your school At least one Letter of Recommendation from
outside your school
4 A personal statement, no more than 500 words, typed, addressing the importance of a COLLEGE
EDUCATION and specifically your views regarding the importance of COMMUNITY SERVICE IMPORTANT!!!
5 Provide a copy of the acceptance letter from the college/university you are planning to attend
