Each prospective Contractor must have the following California Contractor’s License: B: GeneralBuilding Contractor and/or C-39 Roofing Contractor, current, active and in good standing with theCalifornia Contractor’s State License Board on the date and time of the Prequalification Questionnairesubmittal is due This Prequalification Questionnaire with all portions completed, including requiredattachments must be submitted

Each prospective Contractor must answer all of the following questions and provide all requestedinformation, where applicable Any prospective Contractor failing to do so may be deemed nonresponsive and not responsible with respect to this Prequalification at the sole discretion of theUniversity Each prospective Contractor must submit an electronic copy, in PDF format, of the

complete submittal of the questionnaire along with required attachments All Contractors that have

submitted a Prequalification Questionnaire will be notified in writing of either successfully or notsuccessfully achieving prequalification status The decision of the University is final and is notappealable within the University of California system

Contractors should indicate throughout their questionnaire what information is considered

confidential and proprietary All information submitted for Prequalification evaluation will be

considered official information acquired in confidence, and the University will maintain itsconfidentiality to the extent permitted by law

6 CONSTRUCTION EXPERIENCE

Submit Project Data for THREE (3) BEST EXAMPLES OF COMPARABLE PROJECTS

successfully completed within the last ten (10) years constructed in the United States ofAmerica, one of which was constructed in the State of California

A comparable project is defined as having a construction cost at the bid date ranging from

$500,000 to $1,000,000 for the projects submitted, and the following example building types:

 Commercial or institutional research laboratory

 Office building combined with the above.

 Working at an existing commercial or institutional research laboratory while the facility remains in full operation.

 Other facilities that contain a high degree of technical/aesthetic complexity.

Such projects should have possessed the following construction challenges:

 Urban site work with limited construction and staging areas

 Repair of buildings requiring proactive and innovative solutions due to noise, dust, and pedestrian traffic while building is occupied and research is ongoing.

 Working in hard to access spaces with stationary equipment such as lifts and conveyors.

 Project requiring proactive and innovative solutions due to unknown and/or unforeseen field conditions.

 Project complexity requiring tracking of multiple functions and phases

 Project complexity requiring critical path construction scheduling to complete on time.

 Complex phasing plan development and execution.

 Limited staging areas.

 Limited access to working area.

Such projects should include these specific components:

 Structural steel framing

 Installation of KEE type roofing system

 Certification with specific KEE roofing system

 Challenging installation areas

 Limited access to facilities

 Enough crew members available for the duration of the project

 Limited change orders within a project

 Working in a coastal environment

 Challenge may be presented in safely moving materials from laydown areas / across public pathways to roof
