U.S Green Building Council LEED Silver EAp1 Fundamental Commissioning and EA3 Enhanced Commisssioning
The commissioning provider (CxP) shall have documented commissioning authority experience in at least two building projects The CxP shall report results, finding, and recommendations directly to the Owner (DSF)
The project is an academic and preschool laboratory building The construction budget is approximately $36,000,000 and the total project cost
of $47,950,000 The project will be bid in two phases: new addition in summer of 2009 and the renovation in summer 2011

The State of Wisconsin, Department of Administration (DOA) , Division of State Facilities (DSF), (hereafter, State) intends to retain the services of a qualified independent
Commissioning consulting firm with a national perspective to provide Commissioning Services for the School of Human Ecology Addition and Renovation, located at the
University of Wisconsin-Madison, in Madison, Wisconsin
The services provided by the successful Firm will include, but not be limited to those
listed for DOA/DSF Cx Level 3 and to comply with U.S Green Building Council LEED
Silver EAp1 Fundemental Commissioning and EA3 Enhanced Commisssioning
Trang 2 Commissioning the systems (per table below) in this facility to verify that all systems reflect the State of Wisconsin Design Standards and Owners Project Requirements; that systems are complete; and that systems are functioning properly upon occupancy;
That facility staff have adequate system documentation and training
The CxP shall provide the following services:

Note: Commissioning documentation shall be prepared using Microsoft Word, Excel, Access and PowerPoint Any drawing shall be prepared using the AutoCAD R2000 or later All computer generated drawings and documentation must be compatible with the Owner’s computer equipment and software All Commissioning material shall be
submitted in electronic form, either in source or PDF form
Provide documentation of expertise and qualifications for the Firm and the individual qualifications of the Firm’s (CxP’s) representative(s) designated as key personnel to be assigned to the State if selected as the successful Firm (CxP) Provide a detailed
response of the Firm’s past commissioning experience
Each qualifications statement shall be treated with confidentiality by the State of
Wisconsin staff There will be no response to inquiries regarding another Respondent’s qualifications statement
A single Firm (CxP) will be selected to provide Commissioning services for this project The primary role of the successful Firm is to develop and coordinate the execution of a Commissioning plan, observe and document performance, and determine whether
systems are functioning in accordance with the owner’s project requirements and in accordance with the contract documents Additionally, the successful Firm (CxP) will assist with problem-solving or resolving non-conformance or deficiencies, though those responsibilities reside with the Contractors and Design Consultants
