This is a Small Business Reserve Procurement for which award is limited to certified small business vendors Only businesses that meet the requirements set forth in State Finance and Procurement Article, §§ 14-501 – 14-505, 14-301, Annotated Code of Maryland, and who are registered with the Department of General Services Small Business Reserve Program are eligible for award Contractor must submit a copy of their Small Business Reserve Certificate with bid.

The undersigned agrees to furnish any and all items for which prices are offered at the price set opposite each item, in compliance with the specifications, terms and conditions of this Request for Quotation, delivered to the destination point(s) within the period of performance specified above or within the number of days specified after date of order The undersigned certifies that the firm submitting this quotation is not debarred from participating in contracts in the State of Maryland or excluded from federal contract awards.

SMALL BUSINESS RESERVE:

This is a Small Business Reserve Procurement for which award is limited to certified Small Business vendors As of October 1, 2012, the following shall define a Small Business:

A certified minority business enterprise, as defined in the Annotated Code of Maryland Section

14-301, that meets the criteria specified under paragraph (2) of this subsection; or a business; other than

a broker that meets the following criteria:

 The business is independently owned and operated;

 The business is NOT a subsidiary of another business;

 The business is NOT dominant in its field of operation; where

o The wholesale operations of the business did not employ more than 50 persons in its

most recently completed 3 fiscal years;

o The retail operations of the business did not employ more than 25 persons in its

most recently completed 3 fiscal years;

o The manufacturing operations of the business did not employ more than 100

persons in its most recently completed 3 fiscal years;

o The service operations of the business did not employ more than 100 persons in its

most recently completed 3 fiscal years;

o The construction operations of the business did not employ more than 50 persons in

its most recently completed 3 fiscal years;

o The architectural and engineering services of the business did not employ more

than 100 persons in its most recently completed 3 fiscal years;

OR

o The gross sales of the wholesale operations of the business did not exceed an average of $4,000,000 in its most recently completed 3 fiscal years;

o The gross sales of the retail operations of the business did not exceed an average of

$3,000,000 in its most recently completed 3 fiscal years;

o The gross sales of the manufacturing operations of the business did not exceed an average of $2,000,000 in its most recently completed 3 fiscal years;

Beginning October 1, 2012, many businesses which previously did not pre-qualify for the

Maryland Small Business Reserve certification my now be eligible.

ELECTRONIC, FAX AND/OR INTERNET COMMUNICATIONS FOR SOLICITATIONS:

Electronic transactions, and/or communication and/or transmission of solicitations, responses, bids, proposals, acknowledgements, awards and award information are permitted for this procurement Communication or transmission of solicitations, responses, bids, proposals, acknowledgements, awards and award information may be accomplished using facsimile (FAX) transmissions to 410-837-5492, or by TCPIP compliant e-mail sent to the issuing office, twallington@ubalt.edu E-mail transmissions may only include text transmission (not HTML) and may include attachments in Adobe Acrobat.pdf®, Microsoft® (MS) Word® (in MS Windows 2000® compatible format), or

MS Excel®(in MS Windows 2000® compatible format), and graphics in jpg (in MS Windows 2000® compatible) format Attachments must not be zipped or compressed.

Any and all facsimile (FAX) transmission, and/or e-mail must include the solicitation number and title on the cover page or subject line In addition, the University may make copies of solicitations, amendments to solicitations, and solicitation attachments, exhibits, etc available on its internet web site at http://www.ubalt.edu/procurement/solicitations.html Such documents will normally be in Adobe Acrobat.pdf®, jpg format, MS Word®, or MS Excel® format.

The University will require confirmation of receipt of all facsimile (FAX) transmission, and/or e-mail transmissions, by return FAX or return e-e-mail (send a response to acknowledge that the

original message was received) Prospective offerors who download the document from the

University’s web site, or receive documents from a source other than the Issuing Office should immediately contact the Issuing Office and provide their name and mailing address in order that amendments to the RFP or other communications can be sent to them Any prospective vendor who fails to notify the Issuing Office with the required information assumes complete responsibility in the event that they do not receive communications from the Issuing Office prior to the closing date

If a prospective offeror downloads a solicitation from the web site, that prospective offeror is responsible for downloading any and all amendments and/or addenda to the solicitation.

The date and time for delivery of facsimile (FAX) transmission, and/or e-mail delivery of a

solicitation and related documents will be the same as the date and time specified in the solicitation for hard copy / paper copy delivery Delivery will be accomplished at the date and time the

transmission is received by the issuing office

BACKGROUND:

1) Price must include delivery of items, measurements and embroidery services Uniforms must be delivered two weeks after measurements/order Please initial to indicate your commitment to comply.

2) The University may request sample of clothing prior to award Sample of the “ub” logo is

attached.

3) The contractor will be responsible for matching the color and style of the logo.

4) All uniform shirts, jackets, sweatshirts and hats to have “UB” logo embroidery located on

left chest side and above pockets if applicable and hat logo on front center Logo will be given to the successful vendor.

5) The successful contractor must do all measurements at the University campus.

6) All uniform sets are to be separated by employee Name and grouped by department shop

names Hangers preferred for ease of inspection and distribution.

7) Contractor must respond to UB’s calls/emails within 24 hours Please initial to indicate your commitment to comply.
