Trang 4A physical examination, medical examination, or clinical examination (more popularly known as a check-up) is the process
by which a medical professional investigates the body of
a patient for signs of disease It generally follows the taking of the medical history - an account of the symptoms as experienced
by the patient Together with the medical history, the physical examination aids in determining the correct diagnosis and devising
the treatment plan This data then becomes part of the medical record.

Trang 5The routine physical examination, also known as periodic health evaluation, annual physical, comprehensive medical exam, general health check, preventive health examination, medical check-up, is a
physical examination performed on an asymptomatic patient for medical screening purposes.
These are normally performed by a pediatrician, family practice physician, physician assistant, a certified nurse practitioner or other primary care provider.
This routine physical exam usually includes the HEENT evaluation.

Patient: Yes, well I cant straighten up easily
Doctor: Could you (3) as far as you can with your knees straight and stop when you’ve had enough?

(5) That’s not much either Now stand up straight again Now first of all, I would like you to slide your right hand (6) _
of your thigh See how far you can go That’s fine Now do the same
thing on the opposite side.

feet steady.

Now first of all, I would like you to slide your right hand down the right side of your thigh See how far you can go That’s fine Now do the same thing on the opposite side.

together just as they are Keep your knees firm Now try and turn both shoulders round to the right Look right round Keep your knees and
feet steady.
