2. Prevent Recurrence;

3. Act on what is within your control; and

4. Meet the organization’s Goals and Objectives

CORRECT the Immediate Problem:

• This is the action that will get operations functioning as it should be

PREVENT Recurrence:

• This is taking actions to assure the problem won’t happen again, here or in other areas

• Correcting the reasons the condition exists

applied in that order !

• What isn’t how it should be; the undesired state,

• What you see, hear, or sense that tells you there is a problem?

– Ask WHY to that problem,

– After Answering that Why, ask Why again, and again
